Even with a large amount of automation, integration, and traceability between your systems, at some point, in some system, a developer is going to have to mark something as done, comment on what was done, etc.

Instead, I would suggest... a #Retrospective :) This event does not have any "silver bullet" solution in the pocket, but allows team to do their best in order to look beyond what they already know and define an experiment or action to try. In agile methodologies, customer can suggest any changes at any point of time/phase and as from Development/QA team we need to be aware and ready to implement those changes (By dev team) and do the testing once it handed to testing team (get the understanding of scope when dev team is working on code). Totally agree from what I have seen! Be sure you keep your agile process, agile! Adapt to what works best for your team and from my perspective I would say always lean towards having the process later and easier for Developers to follow but does not get in their way of making progress. I have seen some teams try to take on agile and leverage all the various facets of it which can be daunting and overwhelming for a team. Keep it light and keep it agile! Not saying that there might be a person that is struggling with productivity in general and for those instances it should be a a case-by-case analysis with one-on-one conversations with the party involved.

The retrospective meeting itself has the purpose of improving the way the team works. In addition to the retrospective, I would consider a quarterly or release review of the processes, anchoring efficiency and value produced. With these two measures, I would evaluate process bottlenecks to make them simpler and with greater capacity to produce value.

The most probable underrated key point of a team's productivity is celebrating success. If you don't celebrate your wins, you minimize your chance to go there again. Whenever the team is losing the purpose and driving force, give them a flashback of what they're capable of and how they nailed the previous goals. Remember to give them examples of their last win, because they already know how to achieve success. Sometimes the engine only needs a little pat on the back to run smoothly again.

When we feel appreciated we try to give our 100%. To keep that in mind, we need to have a daily connect with team for at least 10-15 mins. On that call we can first start with appreciating individuals(give them rewards) who are giving their best, by doing this we encourage others to give their best and in second half we can discuss about the issues facing and how can we resolve them. Sometimes, we see an issue so big and difficult but in reality it’s not that big and all we need is to see it from different angle or point of view.
