🌟 Exciting News! 🌟 We're thrilled to announce that Avow Care Services has been selected by the Centers for Medicare & Medicaid Services (CMS) to participate in the new Medicare Dementia Care Model! 🧠💙 Under the GUIDING an Improved Dementia Experience (GUIDE) Model, Avow will join nearly 400 participants nationwide to enhance care coordination and support for people living with dementia and their caregivers. 🌐👫 "CMS is excited to partner with Avow under the GUIDE Model," said CMS Administrator Chiquita Brooks-LaSure. The GUIDE Model aims to support both patients and caregivers, helping them stay in their homes and communities. 🏡❤️ We started on July 1, 2024, and this model provides: - Comprehensive, person-centered assessments and care plans 📝 - 24/7 access to a care team member or helpline 📞 - Respite services to support caregivers 🌟 "Our acceptance into the GUIDE Model emphasizes Avow's commitment to excellence in dementia care," said Jaysen Roa, President and CEO of Avow, Inc. Reflecting on her internship at Avow, Natalia shares, "My experience with Avow has been phenomenal! Although it's bittersweet that tomorrow marks the end of our internship, I am filled with hope about potentially joining their nursing team in the future and contributing to their meaningful mission. While nursing school briefly touched upon the topics of hospice and palliative care, this internship has provided me with invaluable insights into the intricacies of these specialized fields. I truly appreciated that Avow not only delved into the nursing aspect, but also acquainted us with the financial, IT, ethical, and various other operational facets of running a nonprofit organization.
