With so many barriers preventing patients from initiating therapy, our Chief Patient Experience Officer Jessica Lens-ter Berg explores why patients need education and personalized support early in the treatment process to ensure better outcomes and refill rates.
Patients may never get started on their prescribed medication, despite their health insurance approving it, despite there being copay or other financial support, and even despite them having the medication in their hands. Patient initiation is not the same as filling or dispensing a medication. It's also the patient behavior after that: the first actual administration. When your 1st refill rate shows a drop-off compared to the initial dispense, consider you not only have an adherence problem, but also that people may have never even started. Many barriers prevent patients from initiating therapy, and they’re often specific to the brand. Additionally, people may struggle with a combination of the following: cost (including cost of logistics like taking time off, transportation, parking, supplies), fear of side effects or complications, already taking multiple medications, unpreparedness, or fear of performing product handling or administration steps, physical limitations, lack of understanding of how the medication will help or even which of their conditions it’s for, patients being unaware that their doctor prescribed the medication and personal beliefs about medication (e.g. better not take (m)any, medications don’t work, etc.). Besides getting/taking the first dose, we should think about the quality of that initiation. Do people start with the necessary knowledge to be successful on this medication? Do they start with the right expectations around symptom relief, treatment efficacy, symptom monitoring, and managing side effects? Do they know how to handle and administer the medication? Are they in the right mindset for this? In summary, did we set people up for successful longer-term use (a.k.a. adherence & persistence)? What can we do about it: Start education and support early: Prepare patients for what is coming, prior to the first dose. Assess what specific challenges, concerns and knowledge gaps exist.
