Delray Medical Center has received the American Heart Association’s Get With The Guidelines® - Stroke GoldPlus quality achievement award for its commitment to ensuring stroke patients receive the most appropriate treatment according to nationally recognized, research-based guidelines, ultimately leading to more lives saved and reduced disability. Stroke is the No. 5 cause of death and a leading cause of disability in the U.S. A stroke occurs when a blood vessel that carries oxygen and nutrients to the brain is either blocked by a clot or bursts. Early stroke detection and treatment are key to improving survival, minimizing disability and accelerating recovery times. Get With The Guidelines puts the expertise of the American Heart Association and American Stroke Association to work for hospitals nationwide, helping ensure patient care is aligned with the latest research and evidence-based guidelines. Delray Medical Center also received the American Heart Association’s Target: Stroke Elite Plus award. To qualify for this recognition, hospitals must meet specific criteria that reduce the time between an eligible patient’s arrival at the hospital and treatment with thrombolytic therapy. In addition, Delray Medical received the American Heart Association’s Target: Stroke Honor Roll Advanced Therapy award by meeting specific criteria that reduce the time between an eligible patient’s arrival at the hospital and treatment to remove the clot causing the stroke. Delray Medical Center also received the American Heart Association’s Target: Type 2 Diabetes™ Honor Roll award. Target: Type 2 Diabetes aims to ensure patients with Type 2 diabetes, who might be at higher risk for complications, receive the most up-to-date, evidence-based care when hospitalized due to stroke. Delray Medical Center also met specific scientific guidelines as a Comprehensive Stroke Center, featuring a comprehensive system for rapid diagnosis and treatment of stroke patients admitted to the emergency department. Congratulations to Delray Medical Center's Tenet Hero Nominee - Carolyn Kern, BS, MPT, Ortho/Spine/Oncology Patient Navigator! The Tenet Heroes program celebrates individuals who make a meaningful impact within their communities and go above and beyond their daily responsibilities. Nominated by leadership and/or fellow colleagues, Tenet Heroes exemplify the company’s core values and commitment to its mission to provide quality, compassionate care in communities we serve. Thank you for your selfless acts of heroism and your commitment to our patients and our community. Read Carolyn's nomination below: "Carolyn Kern began her career at Delray Medical Center in 1996 as a Physical Therapist in the Rehabilitation Department of Pinecrest Rehab Hospital. Recognized for her unique ability to connect with patients, Carolyn was promoted into the Clinical Coordinator/ Navigator Role for the Orthopedic and Spine service lines. In this role, Carolyn designed and created new patient education materials and a folder, to provide potential patients with all of the information they needed when considering joint replacement or spine surgery. The folder was also especially helpful to office managers and the physicians on staff, who shared them with their office patients as well. Once the packet was complete, Carolyn’s tireless dedication to continuous quality improvement led her to make a patient education video on her own time, outlining a script at home, complete with a power point presentation to be posted on the hospital’s website. Last year, Carolyn was asked to assume the role of Oncology and Lung Navigator. She embraced this new challenge as she does with all things. She spent countless hours educating herself on the oncology and thoracic service lines, made appointments to meet with physicians and shadowed our network oncology administrator for guidance. She even worked hard to perfect the call center script so that she could accurately address all of the patient’s questions and needs. Once again - as she has done many times, Carolyn created her own materials for the Oncology and Thoracic programs including a lung cancer flier and prescription pad. She attends all hospital community outreach events to educate the community on their risk for lung cancer and the services available at Delray. Outside of her hospital duties, Carolyn is a strong advocate for voting rights. She was instrumental in bringing in an onsite voter registration booth to the hospital for employees who were unable to sign up because of challenging schedules. Carolyn is the epitome of compassion and humility. Even with her own mother in the hospital, Carolyn found time to round on others’ family members, taking them blankets and water and checking in several times a day. Her mantra remains “what can I do to help” every time you see her. We are #DelrayMedicalCenter. #strongertogetherforyou #pbhnproud #palmbeachhealthnetwork #tenet #tenetheroaward #tenetheroes
