EyeSouth hosted a continuing education meeting for over a hundred Atlanta-based optometrists, where I had the privilege of delivering a lecture on Interventional Glaucoma, with a focus on goniotomy. I enjoy sharing my knowledge because I believe in a collaborative approach to managing glaucoma. As glaucoma cases rise, the workload for specialists becomes overwhelming. A unified effort, bringing together optometrists, comprehensive ophthalmologists, and glaucoma specialists, is essential for delivering optimal care. Co-management plays a crucial role in this approach, improving patient access and preventing burnout by distributing the workload among providers. Fostering open communication, promoting teamwork, and educating on early procedural intervention are vital aspects of this collaborative effort. My presentation delved into the evolution of goniotomy, illustrating how we are now in the midst of a goniotomy revolution. I emphasized the significance of embracing an interventional mindset in glaucoma management and showcased how goniotomy seamlessly integrates into co-management strategies. Fun fact: The spatula for trabeculotomy, described in 1969, bears a striking resemblance to a periodontal knife used in tooth extraction (See picture)! Every patient deserves exceptional visual outcomes, even those with glaucoma. Recently, we successfully treated and co-managed, alongside a referring optometrist, a patient with a significant cataract and preperimetric glaucoma. Choosing FLACS, a Panoptix multifocal intraocular lens, and the Streamline Surgical System, we aimed to achieve an enhanced range of vision and reduced drop burden. The Streamline’s minimally invasive approach and implant-free design facilitate precise viscoelastic delivery into Schlemm’s canal, particularly advantageous for premium cataract patients. This approach can decrease the risk of postoperative hyphema, leading to faster visual recovery.
