    It's time ⏰ to unpack the difference between Social Drivers of Health (#SDOH) and Health Related Social Needs (#HRSN). While related, and often used interchangeably, there is an important distinction to be made between the two...📝 💡 #SDOH are the conditions in which people are born, grow, live, work, and age that impact health outcomes, such as socioeconomic status, education access, neighborhood/physical environment, and more. 💡 #HRSN are the immediate needs that affect people's ability to maintain their health and well-being, such as housing stability, food insecurity, access to reliable transportation, and more. Here's an example to help illustrate the difference: 🔎 SDOH = Individual lives in a neighborhood classified as a food desert. ✅ The intervention = Building a grocery store in that neighborhood. 🔎 HRSN = Individual has low food security. ✅ The intervention = Enrolling that person in a food delivery program At Unite Us, we partner with organizations that address both upstream and downstream needs to reduce health disparities and foster healthier communities.     The Unite Us team took on Nebraska last week with CyncHealth and United Way of the Midlands for the second annual Social Care Open Forum! 🎉 Hosted by Cynchealth and held at United Way of the Midlands’ beautiful office space, the forum brought together key healthcare organizations, community partners, CBOs, and other stakeholders impacting the social care ecosystem in #Nebraska and #Iowa. The forum was jam-packed for CEO Daniel Brillman and R&E Director Gillian Feldmeth, who co-presented on “Improving Health Through Cross-Sector Collaboration,” discussing strategies and insights for: 🚀 Scaling social care infrastructure ✅ Generating real impact and results Dan and Gillian teamed up again for a panelist discussion with our partners, Stephanie Clark, Manager of Ambulatory Social Work at Children's Nebraska, and Diana Strahan & Community Health Alliance Director at Hawkeye Area Community Action Program.     👏 Unite Us is thrilled to show our support for the draft 2024-2030 Federal Health IT Strategic Plan that was recently published by the Office of the National Coordinator for Health Information Technology (ONC) within the U.S. Department of Health and Human Services (HHS)! Developed in collaboration with more than 25 federal organizations that regulate, purchase, develop, and use #healthIT to deliver care and improve patient health, the draft plan "outlines goals, objectives, and strategies the federal government will pursue to improve health experiences and outcomes for individuals, populations, and communities while also promoting opportunities for improving health equity, advancing scientific discovery and innovation, and modernizing the nation’s public health infrastructure." Not only does the plan emphasize the need to further enhance #community care delivery, but it also highlights that a robust, secure IT infrastructure for data sharing is critical to advancing research and analysis and improving #healthequity. Food security, housing security, transportation, and child care are all critical to the health of our communities, so we’re making sure people can get connected to the right help for them. With this investment, we’re ensuring that Albuquerque families have better tools to access the resources they need," said Mayor Tim Keller.
