The answer to yesterday’s #ClinicalChallenge questions is: C. In a large study of adult patients undergoing emergency tracheal intubation, use of an introducer was associated with a higher first-pass success rate compared with no introducer (in most cases a tracheal tube with a stylet). First-pass success rates were higher using an introducer regardless of the method of intubation (ie, direct versus video laryngoscopy) or where intubation was performed (e.g., emergency department, intensive care unit, prehospital). An introducer is most effective in patients with a limited laryngeal view (i.e., Cormack-Lehane III or IV view). In the TRANSMET trial, 94 patients with liver-only unresectable colorectal metastasis were randomly assigned to liver transplantation (LT) or no LT after a median of 20 cycles of chemotherapy. In preliminary results available in abstract form only, LT improved overall survival, and 40 percent of LT patients were ultimately disease free. Current LT protocols require patients to have stable liver disease on therapy for at least 6 months and no extrahepatic disease for at least 12 months. A recent systematic review that included nearly 2,000 adolescent and young adult competitive runners found that female runners sustained more injuries than their male counterparts. All runners, but particularly females, with risk factors for relative energy deficiency in sport (REDS) experienced higher injury rates; athletes with weak hip and thigh muscles were at increased risk of developing anterior knee pain (e.g., patellofemoral pain).
