Advocacy for safe implementation of new technology while maintaining a balance to continue to innovate is an important step to bring new technologies such as #AI to our patients and clinicians. In a letter to Rep: Ami Bera, American Society for Gastrointestinal Endoscopy (ASGE) highlights the significant potential #AI holds for improving diagnosis and treatment of GI cancers through advanced imaging and early detection techniques. The letter underscores the need for continued dialogue on policy to ensure responsible AI use in healthcare and highlights the crucial role of reimbursement strategies and role of medical societies and clinicians in facilitating this discussion. The letter also mentions American Society for Gastrointestinal Endoscopy (ASGE)'s efforts to foster discussions on the ethical use of #AI in healthcare and provides an outline of changes needed to further integrate AI into clinical practice, driving innovation and excellence in digestive care. link to the letter on the post below for further reading.

Teamwork really does make the dream work. Healthcare is all about collaboration. Sharing a few observations that contribute to University of Rochester and Golisano Children’s Hospital At Strong's leadership in innovation. Healthcare is inherently risk averse-- "first do no harm." All too often, this translates to "don't change anything." Hence a our healthcare system stays mired in inefficiencies, and with a rapidly changing workforce, paradoxically falls further and further behind. Large academic health systems tend to be blessed with more funding, faculty, and staffing. They also tend to be loaded with bureaucracy. While their excellent outcomes and cutting-edge research make them beacons of quality healthcare nationally, their model is not replicable for the typical community or regional hospital -- systems which are struggling with workforce turnover, soaring labor costs, thin management, and a margin (often negative) dependent upon clinical volume and efficiency. These small to medium-sized hospitals care for the vast majority of US patients, and they don't have the time or bandwidth to research new solutions. URMC is a mid-sized academic system, and their primary focus is on patient care. They feel the same deep pains as the average hospital, yet still have an academic base eager to research and innovate (especially on the clinical operations side). This combination makes them ideal leaders to drive the pragmatic change that healthcare needs. With less layers of bureaucracy, and less encumbrance from historic and all-too-often isolationist professional siloes (Nursing, Physicians, Pharmacists, Techs), URMC leadership is more agile, and able to look at healthcare delivery holistically, from the point of view of the patient. It's not just about what each profession does, but rather about how they all work together. Golisano thoughtfully picked their starting point for Elemeno's customizable just-in-time microlearning app. High-complexity, high-volume department; leaders desiring change; and a team excited for interprofessional collaboration. A personal highlight was hearing from new Periop Educator Shelby Mancuso how honored she was to be working directly with Surgeon-in-Chief Marjorie Arca. No siloes. No hierarchies. Just teamwork. With their success internally, the Surgical team was then the first to collaborate with Elemeno on a novel best practice sharing network. Many of Golisano's best practices are now functioning as templates to help hospitals around the country support their own staff and drive efficiency and quality. Every hospital joining in the network is benefiting from the shared knowledge of the community. No more reinventing the wheel. Exponentially growing teamwork. Exponentially growing value. Many thoughts as Medicare Advantage continues to come under scrutiny. - MA has features of a value-based program, but most of the current "value" is being delivered through diagnosis capture and complexity payments. Ironically, much like FFS, it pays more to keep people sick (or make them look sick). Don't think that's anyone's definition of value. - To be fair, most doctors (especially specialists) don't do a great job of capturing diagnosis and patient complexity beyond their immediate area of expertise. (We're especially bad at this in MSK). We should have accurate representation of a patient's health in the medical record. Complexity and co-morbidities affect treatments and outcomes. We don't yet have a good way to account for this complexity -- and it's becoming clear the MA system isn't it. - EMRs make all of this worse. It's both too easy and too hard to record, change, or resolve diagnoses. EMRs should facilitate accurate complexity capture and treatment personalization. The goal shouldn't be to increase billing, it should be to increase sophistication and precision. - As MA scrutiny grows, the business model of many alternative care models (especially in primary care) will be challenged (potentially dealing another blow to retail healthcare). - CVS' purchase of Signify health is put into full context by this article. - It boggles the mind that CMS can develop and administer a program that nets billions for inscos (at the expense of taxpayers) while reducing physician reimbursement yearly and rolling out ineffective VBC programs for providers. CMMI should have an open-door policy for anyone delivering care and working on true value-based models. There is plenty of evidence pointing to this fact: provider prices are driving the majority of the increases in premiums and costs, especially in Massachusetts where health care costs are among the highest in the nation. Moreover, the existing health system is not designed to advance health or wellness, but rather sick care. At Wellpoint Massachusetts (formerly UniCare State Indemnity Plan (becoming Wellpoint)) we will continue to partner with physicians and providers to advance models of payments and health services that incentivize our members to engage in their whole health, live healthier lives, and access health services affordably- as best as we can. However, it's crucial that employers and taxpayers actively demand a more transparent and competitive health system that's fair to premium payers in the private market.  Ultimately, it's premium payers who subsidize government health programs and pay for exorbitant provider prices. I'm working with Lyssa Limbrecht-Moss from Optimal 340B and we've determined we don't need any files from the CP to do an audit. We just take one audit download of all NDCs from the TPA and one EMR download from the hospital and we can audit every NDC and dispense for ALL of a hospitals CP for $1,000 total (plus one 3K implementation fee).  Audit is just verifying valid patient status, eligible NPI and location.  No Medicaid check because the TPA isn't accumulating anything when the payor is Medicaid. Doesn't matter if chain pharmacies or all different because we don't get any data from CP directly.  And requires zero implementation or connection to CP. Be SURE (and cost effective) with 340Bsure
