This 20-year follow up report reveals that while rates of uninsured have declined, we have not yet reduced health disparities in the United States. Until we recognize the multi-layered impact of community assets, politics/legislation/health policy, systems of care, healthcare provider and individual factors and accordingly implement multi-level approaches with measurable accountability across each layer as well as make visible the invisible, change will continue to move slowly.

Today I'm both grateful and wondering how NPR, KFF Health News and the Newsom Administration missed an opportunity to promote the state TA Marketplace giving CBO's FREE expertise to sustain and scale their services. Just Whole Care is helping CBO's just like the ones named, provide and sustain intensive care coordination (aka ECM) and asthma remediation - as well as other services addressing physical, behavioral, and essential needs through integrated, equitable, trauma-informed care, impact measurement, and systems change. It's ALL thanks to the TA Marketplace (TAM) neglected by this piece. We're talking small, Black-owned CBO's leveling up with measurement-based, equitable care and new payment methods near overnight; at least by Medicaid standards. The TAM will be critical in light of difficult budget cuts the Administration proposed last week to maternal child health, behavioral health, and public health. (Thoughts to come as details arrive.) So let's make sure we and the media ALL give credit where it's due. The California Department of Health Care Services under Secretary Ghaly, Directors Baass and Sadwith, and leaders like Palav Babaria and at PCG’s Health team are CRUSHING it - resourcing and marshalling #healthequity expertise for diverse, marginalized California communities. Indeed a turning point for women's health. I am not a political person, but this is something I can proudly support. Very proud of my friends Liz Powell, Jodi Neuhauser, and Candace McDonald for launching this Women's Health PAC who have the support of thousands of women in the healthcare industry. With more money, research, information, innovation, funding, and focus on women's health... everyone wins. Including men. This is not exclusive, this ensures inclusion of all people, especially women who have been excluded from most of the above including in healthcare. New study in JAMA open highlights the unique challenges faced by physicians with disabilities (PWDs) in healthcare. I was privileged to lead this article with the Association of American Medical Colleges (AAMC) workforce team (Michael Dill, Sarah Conrad, Zakia Nouri, Xiaochu Hu) and Zoie C. We all have seen by now that AI holds the promise of reshaping healthcare delivery, and this goes particularly for marginalized communities. Its widespread adoption, however, necessitates careful navigation of potential biases and ethical concerns to ensure equitable outcomes for all! First of all, I applaud the nurses for standing up and making their voices heard. I get it. I do. There is much fear about bots, automations, and clinical decision support. Those of you who have read or seen my other posts know that I am ringing the bell and raising the alarm. We are experiencing a critical shortage of healthcare professionals, and it’s only going to get worse. The industry needs to get better, faster, and more efficient—fast. Clinicians (and other healthcare workers) should not fear that AI is taking away their jobs. They should fear those who know how to work with AI. AI isn’t coming. It’s already here. In a world where the thin line between AI and general intelligence blurs very quickly, we see healthcare as the next logical frontier. AI does not have to be perfect, but it does have to be better than a human to add value. It seems reasonable. It is, after all, an assistant. It’s not meant to do everything but augment and assist professionals so they may operate at the top of their license. Spell checking? Grammar checking? If you've ever spent more than 5 minutes around me, you know that I'm deeply committed to the realization of a learning health system (LHS)--a virtuous cycle where healthcare delivery informs research, and research beneficially improves care delivery. Great summary, Reed Omary, MD, MS, of some of the major factors impeding action on climate and sustainability in healthcare (1. not our business; 2. costs money and 3. we are too busy). The major question that has occupied me since you and I spent time together in Sundance (at a great climate and health conference organized by Vivian S. Lee, MD, PhD, MBA - see https://lnkd.in/gdw6nsSE) is what needs to happen to create a tipping point in which rapidly accelerating positive action feeds on itself. Healthcare is still largely a cottage industry, but maybe this is as much an opportunity as it is a challenge. One thing we know is that healthcare employees want to see their employers act (see https://lnkd.in/gg5Xt5QU). I think that may be one of the biggest leverage points ... !

"It Is Time to Consider More Price Regulation in Health Care." My favorite excerpt below: Research on hospital productivity in Medicare finds that hospitals often misallocate their resources, and the hospitals that spend less may have outcomes that are as good as the hospitals that spend more. In the commercial market, hospitals paid higher prices do have better mortality outcomes, but that is only true in less concentrated markets. In markets with little competition, hospital prices are high but quality is lower than in competitive markets. I have been a happy One Medical customer for a long time, and what they have done to make instant care more accessible is impressive. But, hang on tight and listen to this story for a sec...🔥🔥🔥 One day, I had a sore throat -- minor, I would say, but I also have a toddler at home, so didn't want it to get worse. I booked the next-available video session with a One Medical provider, and ten minutes later, I was speaking to someone over Zoom. Pretty impressive! We talked about my sore throat symptoms (typical stuff), and the provider suggested I take over-the-counter ibuprofen -- nothing else. He ordered a strep throat test just in case, but that turned out negative. So, all-in-all, a minor sore throat with minimal intervention. Total time on the call: 8 minutes. One week later, I got the bill. What's your guess for how much One Medical charged for the eight-minute virtual visit? 🤔 If you guessed the equivalent of $2,536.38/hour for the call, then you're right! I emailed the provider to ask if he knew what One Medical was billing patients for virtual visits with him, and he didn't. I sent emails to two different people in the Billing Department saying that I found the charge concerning, but they said the charge was correct and refused to make an adjustment. Finally, the case was escalated to a District Manger, but he side-stepped the issue and blamed the insurance company for not being willing to cover more of the outrageous charge. When I responded that my concern was not with the proportionate share of the charge covered by insurance but, rather, One Medical's exorbitant (unethical?) charge itself, he refused to respond, and that was the last I heard from One Medical. Long story short, THIS is what's wrong with healthcare in America right now. Companies like One Medical/Amazon are giddy, talking about how they are reducing friction and improving access using technology, and on the flipside, the charge for such care could not be any more prohibitive. If you wanna make care "frictionless," you need to provide it for a price that is reasonable and, dare I say, ethical. None of us “enjoyed” the additional authority we needed during a public health emergency, and without this authority we would have struggled even more than we did. We learned a lot about that authority and we reaffirmed what is necessary to protect public health outside of an emergency. Advance care planning conversations save money and increase utilization of specialized end-of-life care (hospice and palliative care). On a personal note, I had been speaking with my in-laws about completing an advance directive. At Christmas one year, we had a direct conversation about the Five Wishes document that they had completed. My Father-in-law Gary was very direct and clear in how he spoke about his wishes; no machines. He fiercely did not want to become depended on others. Gary had a heart attack a year later. He was kept alive on a ventilator until my husband could fly in and say goodbye. We were all so grateful to have had that conversation. There was no second-guessing. He was clear in telling us his wishes and so we were clear in honoring them. ACP conversations are VITAL for practitioners and family members to have in order for people to have the ending they want, or hope for. (Gary died before the launch of The Death Deck or we would have also used it to help have these important conversations).

Humanizing healthcare through collaboration. In my approach, I'm deeply committed to igniting change at its core. Challenging the status quo isn't easy, but as a bold leader, I'm always eager to embrace the challenge. My aim is to collaborate with the right great minds in the industry , like Alvin Gore, MD leading the charge at UM and collaboration in care through coding CDI and providers. In addition to Cesar M. Limjoco, M.D. , to revolutionize patient care through innovative practices and insights for Sepsis and quantitative fallacies in diagnosing. I am inspired by Zeev Neuwirth, MD "Reframing Healthcare," I'm driven to unravel the legacy framework that hinders efficiency and quality, aiming to reshape healthcare for the better. Dr. #Zeev puts it so well, “our brains are wired in a legacy framework that is engrained in higher cost, lack of efficiency and quality of care for our patients”. Together, let's embark on this transformative journey, rallying all clinicians, & all healthcare leaders, to join the movement towards a more compassionate and effective healthcare system.
