### 🚨 Ulnar Collateral Ligament/UCL/“Tommy John” Ligament 🚨 We are “full swing” into baseball season and, unfortunately, that means injuries, especially to pitchers. You may have heard about the ulnar collateral ligament (UCL), often referred to as the “Tommy John” ligament. This injury has become an epidemic among major and minor-league baseball players. More concerningly, over the last 20 years, it's become a significant issue in college, high school, and even youth baseball! **Prevention is key** and, thankfully, these injuries are preventable. However, when they do occur, we are always searching for less invasive, non-operative ways to biologically heal the ligament and avoid Tommy John surgery. **Meet Peyton** – a non-operative success story after a UCL injury. We've found that adding bone marrow cells in addition to PRP (Platelet-Rich Plasma) tends to result in better outcomes for our patients. Shoulder health is all about rhythm! The shoulder joint, shoulder blades, and collar bone all work together to produce strong and balanced/coordinated movement. The shoulder joint and shoulder blade should move at a 2:1 ratio. For instance, after about 40 degrees of shoulder abduction (arm moving away from the body), the shoulder blade should follow with about 20 degrees of lateral/rotational movement. The collar bone acts as a strut and provides support (it moves as well). When this rhythm is off, cycle times to injury occurs quicker (i.e. frozen shoulder, thoracic outlet syndrome, scapula winging, scapula dyskinesis, etc.). For shoulder health, incorporate exercises that activate, reinforce, and strengthen the shoulder in all planes of motion. Try this exercise to keep your shoulders healthy and on rhythm. Vertical femoral tunnels may lead to continued pivoting despite intact ACL graft. Such pivoting may lead to accelerated cartilage and meniscus degeneration. If your button exits just proximal to patellofemoral joint and is intrarticular, you probably drilled a vertical tunnel.
