Description

The Assistant Director of Case Management works with the Director in coordinating and directing staff and case management activities assuring effective use of available hospital and community resources and monitoring patient outcomes. Assistant Director of Case Management is responsible for the coordination of patient care through multidisciplinary collaboration with medical, nursing and ancillary departments, assists with the planning of patient care including, but not limited to, facilitation of patient progression and proactive discharge planning. The Assistant Director will ensure Case Management activities are consistent with the hospital’s mission, case management standards, objectives, and bylaws. The Assistant Director is responsible for the CQI activities of the department and ensuring all staff members attend required inservice education programs. Has full access to patient health information.

Requirements

  • Currently licensed as a Registered Nurse by the North Carolina Board of Nursing. BSN preferred.
  • Certification in Case Management preferred.
  • Minimum of 5 years of clinical experience in a hospital setting.
  • Minimum of 5 years of case management, utilization and/or discharge planning experience required.
  • Excellent organizational and communication skills required.
  • Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time. Must be able to lift and carry up to 30 pounds.
