Care Indeed is hiring a compassionate and motivated Associate Client Care Manager to provide top-quality service to our clients. Strong management, leadership, and problem-solving skills and a willingness to learn and continuously improve are essential for success in this role. A demonstrated background in education, skills, knowledge, and experience in aging and care management is also necessary to provide quality care management services.

Duties and Responsibilities

CARE COORDINATION

  • Coordinate with various stakeholders to ensure client care plan is accurate, up to date, and meeting the needs of the client
  • Coordinate and attend client care conferences

CLIENT SATISFACTION & RETENTION

  • Receive caregiver and point of contact reports and requests and make sure they are documented and acted on
  • Follow up with client and their point of contact to make sure any concerns have been addressed in a timely manner
  • Communicate with client and point of contact on a regular basis regarding the care plan and client or family needs
  • Build and nurture relationships with clients and their point of contact through regular contact, communication, and in-person visits
  • Provide quality assurance visits and check-ins DATA MANAGEMENT
  • Monitor re-assessment deadlines
  • Document client interactions and pertinent caregiver reports
  • Monitor assigned accounts
  • Other duties as required

Education and Experience

  • Bachelor's degree in psychology, counseling, rehabilitation, gerontology, nursing, or sociology required.
  • Case management: 1 year (Preferred)
  • Senior care: 1 year (Required)
  • Strong presentation skills with the ability to effectively communicate the value of Care Indeed and its mission.
  • Proficiency in Microsoft Office (Word, Excel, PowerPoint, and Outlook).
  • Availability during and after business hours, and for on-call rotation on weekends.
  • Excellent written and verbal communication skills with an eagerness to learn.
  • Unwavering commitment to ethical work standards in all interactions with clients and employees.

Work Hours & Benefits

  • Monday to Friday, 9:00 AM to 5:30 PM; days may be flexible based on the need
  • Medical, Dental, Vision insurance (waiting period is 60 days from hire date)

  • 401k with matching (After completing 200 hours and six months of employment)
  • You will be reimbursed for mileage and out-of-pocket expenses for any company-related initiatives if you use your own transportation.
  • You are entitled to accrue 24 sick hours of paid sick leave, which you can use after your 90 days, to be allocated for illness or personal business each calendar year in accordance with our sick leave policy.
