Intern Position Description
Position Responsibilities: The Intern works closely with the Clinical Manager to facilitate home visits and provide evidence-based interventions to consumers in our catchment area. The intern is expected to conduct home visits and complete behavioral health assessments and evidence-based interventions to older adults throughout the AgeSpan catchment area. In addition, the intern will be required to complete care coordination and documentation during business hours. Hours may be flexible but will generally be between 8am and 4pm.

The intern is expected to conduct home visits and complete behavioral health assessments and evidence-based interventions to older adults throughout the AgeSpan catchment area. In addition, the intern will be required to complete care coordination and documentation during business hours. Hours may be flexible but will generally be between 8am and 4pm.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
- Provide behavioral health assessments and therapeutic interventions to a case load of 5 to 6 older adults &/or family members
- Conduct in-person sessions in the client's home
- Complete clinical documentation for each therapy session
- Provide resources and referrals, collaborate with other professionals at AgeSpan and in the community
- Conduct risk assessments, suicide assessments, and crisis intervention as needed
- Create individualized treatment plans according to client needs and circumstances
- Attend weekly supervision as scheduled
- Participate in in-service trainings, learning modules/courses, and workshops to help develop additional skills
- Able to work independently
- Display strong communication and advocacy skills
- Experience in case management and/or mental health counseling
- Preferred experience and interest working with older adults
- Valid driver's license and reliable transportation required
