Job Summary
This position is in the Department of Aging and Community Living (DACL), Clinical Division. The mission of DACL is to advocate, plan, implement, and monitor programs in health, education, employment, and social services which promote longevity, independence, dignity, and choice for the city’s older adults (ages 60 years or older), people living with disabilities (ages 18-59 years), and caregivers.
The incumbent serves as Case Manager responsible for assisting clients and their care givers with evaluating and coordinating social service options, determining needs, and monitoring and tracking the efficacy of all services provides to clients to ensure program goals are met.
Duties And Responsibilities
Provides needed crisis intervention services for identified clients. Facilitates and ensures collaboration among all agencies, providers and individuals connected (and/or providing services) to assigned clients to ensure effective communication and appropriate overall coordination of care. Assists subordinate Case Managers and Supervisors in responding to hearings to include researching and auditing case files, evaluating decisions, and taking corrective measures, if warranted. Works collaboratively with staff, clients and involved agencies, providers, and individuals to conduct assessment and develop service plans that will result in the mitigation of barriers, address client needs and promote self-sufficiency.
Connects assigned clients to, and ensure their engagement in, needed supportive services that will address barriers and challenges they face. Tracks client engagement/participation in supportive services. Monitors and evaluates the efficacy of services that clients are engaged/participating in. Completes all required case notes, service plans, documents, files, and assigned reports related to clients/case activity.
Qualifications And Education
Degree: Behavioral or social science; or related disciplines appropriate to the position.
or
Combination of education and experience that provided the applicant with knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
or
Four years of appropriate experience that demonstrated that the applicant has acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.
and
Specialized Experience: Specialized experience is experience which is directly related to the position and has equipped the individual with the knowledge, skills, and abilities to successfully perform the duties of the position. Specialized experience includes working with an adult population in a home, community or hospital or long-term care setting, working with clients and their care givers evaluating and coordinating social service options, determining needs, and monitoring and tracking the efficacy of all services. To be creditable, the incumbent must possess at least one (1) year of specialized experience equivalent to the next lower grade level in the normal line of progression.
License and Certifications
Must have a valid driver’s license.
Work Conditions/Environment
Some work is performed in a normal office setting, co-located within community organizations. Work in the community may include private homes and apartments, offices, institutions, potential housing, meetings with providers, and professional training sites. The work also requires incumbent to drive to various sites when conducting home visits, attending professional meetings, and providing social service assistance at various locations.

Collective Bargaining Unit (Non-Union): This position is not in the collective Bargaining Unit.
Position Designation(s): This position has been designated as Protection Sensitive, therefore the incumbent of this position will be subject to enhanced suitability screening pursuant to Chapter 4 of DC Personnel Regulations, Suitability and as such, incumbents of this position shall be subject to criminal background checks, background investigations, and mandatory drug and alcohol testing, as applicable.
Emergency Designation: This position has been designated as Emergency. Employees occupying positions designated as Emergency are required to provide advice, recommendations, and/or specific functional support necessary for the continuity of operations during a declared emergency. Remain at their duty station, or alternate work location (approved by their supervisor), if activated, when a situation or condition occurs and results in early dismissal for nonessential/non-emergency employees. Report to their duty station, when activated, on time and as scheduled when a situation or condition occurs during non-work hours, and results in the late arrival or closing of District government offices for non-essential/non-emergency employees. Telework during a declared emergency, instead of remaining or reporting to his or her duty station, if directed by the agency head (or designee), supervisor or manager.
