Summary: The community aging planner coordinates both internal strategic and external community planning, sharing related communications and conducting research to inform efforts. This position involves strategic planning, fostering collaborative relationships with team members and partners, research and development of new business initiatives, and coordination of strategy implementation.

 

Essential Job Functions

Plan Development and Implementation (40%)

·     Coordinate SourcePoint’s 3- to 5-year strategic planning process, including providing support, direction, and acting as liaison among consultants, board members, stakeholders, the executive team, and members of leadership.

·     Coordinate the Delaware County Age-Friendly initiative, ensuring ongoing planning, progress toward community goals, continued pursuit of an age-friendly community, and maintenance of age-friendly designation.

·     Conduct community-based research and collects data to guide all planning decision making, including regular engagement with AARP’s livable communities network.

·     Assess impact and demonstrate outcomes of strategic and age-friendly plans through monitoring and reporting of key measures and accomplishments at least semi-annually.

 

Community Awareness, Partnership Building, and Maintenance (30%)

·        Serve as an expert in the aging network with a focus on services, programs, and the World Health Organization’s age-friendly community initiatives, including engaging with events, trainings, and meetings to promote advancement of aging issues.

·        Develop and maintain community relationships through engagement in community initiatives, task forces, and regional aging initiatives to guide components of strategic and age-friendly plans.

 

New Business and Community Initiatives (20%)

·        Continually analyze internal and external data trends to advise the executive team on new business initiatives; identify gaps and shortages in services within the county and work with leadership and the community on recommendations in addressing those gaps.

·        Utilize research and data to address service gaps to meet the ever-changing needs of the older population in Delaware County by providing recommendations and assistance to leadership in new business initiatives.

·        Expand community relationships to support new initiatives including creation of community subcommittees, as necessary, and persuading partners to integrate age-friendly concepts into other community enhancement plans.

·        Research and support leadership in pursuit of new funding opportunities to support new or growing programs and service offerings in line with agency plans.

 

General Responsibilities (10%)

·        Actively participate in SourcePoint required meetings and events.

·        Pursue and attend training in line with professional development in the role.

·        Maintain a thorough understanding of the programs and services provided and supported by SourcePoint. This includes an understanding of SourcePoint programs, as well as programs supported through partnerships.


Education and Experience

·        A bachelor’s degree in strategic or community planning, public administration, public health, social work, human services, or related field required.

·        Excellent communication skills, both verbal and written.

·        Exceptional interpersonal and relationship management skills.

·        Strong organizational and time management skills.

·        Experience and ability to provide public speaking to a variety of audiences.

·        Minimum of two years’ experience in professional practice.

·        Experience with human service programs, specifically those designed to promote healthy aging.

·        Knowledge of age-friendly communities, as well as an understanding of emerging issues, trends and challenges involving the older adult population.


Essential Physical Requirements/Working Conditions

·        Prolonged periods of sitting at a desk and working on a computer or phone. 

·        Must be able to drive and accommodate a flexible schedule for off-site meetings and special events, including occasional evenings and weekends.


Other

It is recognized that in any organization, particularly a small organization, it is necessary to assume new responsibilities appropriate to ensure a smooth continuity of operations with the organization. Not only is the community aging planner required to wear many hats, but it is expected that this person will be familiar with the work of others to the point of being able to fill in on a temporary basis. It is also anticipated that other organizational needs will emerge from time to time which will be assigned to the planner.
