If so, Health Connect America is looking for a dedicated and compassionate Community Support Specialist to join our team. As a Community Support Specialist, you will play a vital role in empowering individuals to achieve their mental health and well-being goals through community-based support and intervention.
What Will I Be Doing:
As a Community Support Specialist, you will be at the forefront of providing essential care and assistance to at-risk youth and their families. Your primary responsibilities will include: Individualized Support, Community Integration, Emotional Support, Skill Building, Crisis Intervention, Documentation, Team Collaboration.
You Will Be Working On:

What Will I Be Doing:

- Educational Initiatives
- A Bachelor's degree in Psychology, Social Work, Counseling, or a related field
- Two years experience working with kids and families
- Compassion, empathy, and a genuine desire to support individuals on their mental health journey
- Strong communication and active listening skills
- The ability to work independently and as part of a team
- Knowledge of community resources and social services
