Description

Under supervision of Project Director and/or Director of Service, responsible for community outreach, education, and direct patient services for the Smoking Cessation Program. Assists patients, in the inpatient and/or outpatient settings, in smoking cessation and relapse prevention, using therapeutic counseling techniques and standardized protocols. Communicates, as needed, with referring physicians and other hospital/clinic staff; conducts site-based Smoking cessation Intervention Program informational sessions and patient support groups, as needed; and performs administrative tasks related to patient care. Responsible for maintaining up to date information on current tobacco trends and medical policies related to tobacco dependence treatment. Collaborate with project partners to maintain records of current policy and practices of project’s participating medical groups. Develop relationships with health care provider organizations (departments, hospitals and clinics). Coordinating with these health care organizations and the medical staff to increase adherence to the smoking intervention best practices. This will include efforts to assure changes in the policy and practice of these health care organizations to maximize utilization of tobacco resources available to the Queens community. Organize conduct, initial and follow-up provider education programs. Coordinate follow-up with individual providers to ensure they have the necessary resources to reach the needs of their client population. Serves as active member of specified hospital committees, task forces, and professional organizations. Accountable for effectively coordinating any project related community education services. Utilizes independent and collaborative judgment in decision-making. Demonstrates leadership and problem solving skills. Incumbent should have skills in teaching techniques that encompass group dynamics. Incumbent should also have the ability to effectively communicate both orally and in writing including presentation skills. As needed, act as liaison with insurance companies to inform the medical community of tobacco related services available and policies in place by these companies.

Responsibilities

  • Provide one-on-one counseling, and follow-up services for patients for smoking cessation.
  • Contact administrative and clinical staff at hospitals and clinics in Queens to schedule group and individual trainings with clinical staff teaching best practices for tobacco cessation.
  • Conduct individual sessions educating providers on counseling and medicines available that can help their patients quit smoking.
  • Work with clinical staff to problem-solve barriers to patients quitting. Given the diversity of Queens, this requires cultural sensitivity to the various ethnic groups represented in Queens.
  • Work with health care provider to identify the resources that would best help the patient population.
  • Provide feedback to the administrative and clinical staff about the implementation of tobacco cessation screening and care. Incumbent will use state tracking tools and any clinic data shared to chart and track progress.
  • Research new articles on tobacco cessation topics from newspapers and medical journals and distribute to the network or providers who are working with Queens Quits.
  • Work with clinical and administrative staff to provide technical assistance with changes in hospital or clinic’s system to incorporate best practices for tobacco cessation.
  • Under Project Director and/or Director of Service supervision, responsible for community outreach, education, and direct patient services for the Smoking Cessation Program.   • Develop relationship with providers to increase awareness and knowledge surrounding smoking cessation. Arrange and conduct both group and individual training events to health care providers in Queens. Serve as resource expert in patient education; maintain and improve the Queens health care organizations’ awareness and referrals to tobacco cessation resources and materials; collects, analyzes and monitors utilization and other project data; prepares and presents reports as required.
  • Maintaining relationships with all project partners: state level as well as local level. . Keep up-to date on the changes within the services and needs of the projects network. Research and respond to providers’ requests for information related to smoking cessation services and policies.
  • Identifying problems related to smoking cessation services and make recommendations for improvement. Attend and participate in meetings and quality improvement activities as required; research smoking cessation and grant related topics, attending related meetings as needed; ability to make quick and accurate assessments of behavioral conditions; assist with patient and provider interviews.
  • Remains informed of current trends in field by reviewing literature, data and attending conferences or training programs in an effort to further mission of the hospital through active support of the strategic goals.
  • Performs related duties as required professionally or as assigned.

Qualifications

  • Bachelor's degree in counseling, nursing, social work, health education, psychology, or related field required.
  • Two to three years experience in patient counseling, motivational interviewing, and/or health coaching.
  • Experience and/or knowledge in addictive disorders therapy, behavioral therapy, and/or pharmacological therapy as related to tobacco dependency preferred.
  • Certified Tobacco Treatment Specialist (CTTS) certification and/or training and experience.

OR

  • Master’s degree with one year of relevant health education experience.
  • Current knowledge of wellness health information and behavioral health assessment.
  • Ability to work with groups and individuals; must be supportive of their needs/concerns and with sensitivity to the underlying changes in their lives.
  • High level of analytical ability required for establishing good rapport with partners and participants of the project in order to accurately identify resources that may help. This ability would also be important to effectively evaluate clinical problems/treatment progress with diverse types of patients.
  • Interpersonal skills at very high level to communicate effectively with physicians, nurses, other health professionals, patients, patients' families.

The Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $58661 - $70000 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
