Qualifications

• Licensed and Registered: A valid PT License and Registration in New York.
• Experience: Proven experience in adult PT.
• Communication Skills: Strong communication abilities.
• Technical Proficiency: Familiarity with EMR software is a plus.
• Homecare Experience: Previous homecare experience is advantageous.
  
  

Responsibilities

• Patient Interaction: Communicate effectively with office staff and patients.
• Scheduling: Coordinate and schedule your visits directly with your patients.
• Care Planning: Evaluate patients and develop tailored plans of care based on their functionality.
• Follow-up: Conduct follow-up visits according to the plan of care and patient progress.
• Documentation: Accurately document visits in our web-based EMR system, adhering to Medicare guidelines.
• Compliance: Maintain active credentials and perform all visits in compliance with HIPAA and company policies.
