Functions as a member of the admissions staff to process patients into the Brook Lane system. Accountable for initial and/or emergency evaluations to assess and triage patients, facilitating entry into Brook Lane programs and referrals to community partners.
Key Responsibilities
Completes psychosocial assessments with patients, parent/guardian and/or key sources to identify problems and symptoms creating dysfunction to identify an appropriate DSM-5 diagnosis.
Effectively triage patient symptoms, risk level, and needs to match them with the most clinically appropriate services, either at Brook Lane or in the community.
Maintains documentation of services provided in compliance with center policies.
Assesses and documents patient’s level of risk using validated screening and assessment tools. Follows the organizations policies and procedures regarding managing suicide risk.
Disseminates information to appropriate persons/departments and answers feedback to referents in a timely manner.
Maintains level of generalized knowledge necessary to professionally answer questions regarding Brook Lane Health Services’ resources routinely asked by clients as well as the general public.
Maintains knowledge of available community resources to connect patient’s with needed services and supports that are not available at Brook Lane.
Exemplifies customer service. Maintains a helpful/positive working relationship with all customers (i.e.: patients, coworkers, other departments, outside agencies, etc…). Maintains an appropriate level of involvement with all customers
Qualifications
Completion of a master of social or behavioral health program from an accredited university.
Licensed at the graduate level (LGSW or LGPC) required to deliver therapy services.
Licensed and certified clinical (LCSW-C), or (LCPC) preferred.
One year experience working with the mentally ill using the DSM-5.
Training or experience in working with specialized populations.
Ability to relate well to a variety of people and situations.
Ability to relate therapeutically to people in crisis.
