Job Summary
The Intake Representative position is responsible for reaching out to CPHL Managed Long Term Care Plan (MLTCP) referrals by phone; carefully explaining plan benefits and enrollment procedures; determining patient’s current services and what they are looking for today; and appropriately scheduling Intake RNs for in-home assessments. The Intake Representative acts as in-house liaison for RNs working in the field, insures that UAS’ (New York State’s clinical assessment tool) are assigned correctly, and tracks RN visit outcomes. Intake Representatives have excellent verbal communication skills , are able to maintain timely and accurate records, and are diligent about any and all follow up. An Intake Representative must have a strong desire to help people navigate and access healthcare, while meeting the high standards and productivity demands of a fast-paced, growing health plan.

Primary Responsibilities
- Outreach to all referrals in a timely manner
- Review and understand referral source and ancillary documentation
- Timely and accurate documentation of all efforts
- Understand and be able to explain long term care program and enrollment procedures
- Excellent verbal communication skills
- Track RN visit outcomes
- Other assignments and duties as needed
Education and Experience :
- HS Diploma or equivalent
- Customer service, advocacy work
- College degree
- Healthcare administration experience
- Excel, Word
- Language - Spanish, Russian or Bengali fluency, preferred
