Health Projects Center (www.hpcn.org) is recruiting an Intake Specialist to provide support to Health Projects Center's Programs.

Health Projects Center is a 501(c)3 non-profit corporation established in 1980. We provide exceptional community-based programs that support elders to live and age with dignity at home. We deliver culturally appropriate programs in both English and Spanish for older adults, family caregivers and health workers.

Health Projects Center serves Monterey, San Benito, and Santa Cruz Counties. We run the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program, and the Central Coast Area Health Education Center. Our programs support over 1,300 families annually to live and age at home at a fraction of the cost of an institution.

Why This Role Is Important To Us

The intake specialist, you'll play a crucial role in our healthcare program by managing various responsibilities. Your duties include collecting and recording comprehensive health information from prospective participants with a focus on accuracy and confidentiality. You'll collaborate closely with the program team to compile data for analysis and reporting. Additionally, you'll provide essential support to our mobile health workers by ensuring the seamless integration of technology and efficient operation of facilities, troubleshooting technology-related issues, and maintaining program-related databases. Your role also involves offering translation services, assisting with administrative tasks, and facilitating communication with diverse clients. Furthermore, you'll coordinate with external partners like Central California Alliance for Health (CCAH), obtaining eligibility lists, contacting referred clients, and ensuring timely submission of necessary documentation. Your proactive approach to collaboration and attention to detail will contribute to the smooth functioning of our intake processes and overall program success.

Essential Functions

  • Health Information Management
    • Collect and record comprehensive health information from prospective participants, ensuring accuracy and confidentiality.
    • Collaborate with the program team to compile relevant data for analysis and reporting.
  • Technology and Facilities Support
    • Provide essential support to our mobile team of health workers by ensuring the seamless integration of technology and efficient operation of facilities.
    • Troubleshoot and address any technology-related issues, contributing to the smooth workflow of our field teams.
  • Database Management
    • Demonstrate proficiency in database management, including data entry, maintenance, and generating reports as needed.
    • Collaborate with internal teams to ensure the accuracy and completeness of program-related databases.
  • Administrative Support
    • Answer phones, respond to inquiries, and provide administrative assistance to ensure the smooth functioning of daily operations.
    • Monitor the program status in Activate Care in changing case manager's status; ex: enrolled, termination, deceased.
    • STFP- Medi-Cal status; renewal/terminations and send report to case managers monthly.
  • Translation and Communication Support
    • Offer translation services as needed to bridge communication gaps with diverse clients.
    • Assist with correspondence, including processing mail, responding to inquiries, and maintaining effective communication channels.
    • Provide support to mono lingual staff in ECM
    • Collaborate with the Intake Coordinator
  • Coordination and Collaboration with Central California Alliance for Health (CCAH)
    • Obtain eligibility list monthly from CCAH
    • Contact ECM clients that are being referred by CCAH
    • E-mail Provider Change Request's when case managers state member's termination status
    • Submit Treatment Authorization Release (TAR/ECM02) to initially enroll or re-enroll a member after their service authorization period has expired.
    • Update Activate Care with the new TAR/ECM02 Medi-Cal information once renewal obtained.
    • Answer CCAH's email's pertaining to member questions as needed.
    • Obtain monthly Medi-Cal current eligibility status and e-mail the case managers the list.
    • Attend the CCAH-ECM office hour meetings, as needed.
    • Sending calendar evite for available time slot to have care manager contact the member.
    • Collaborate with case managers as needed.
Qualifications

  • At least two years of relevant work experience.
  • Spanish language fluency required.
  • Strong proficiency in Microsoft Office suite, with demonstrated skills in maintaining spreadsheets and databases.
  • Outstanding verbal and written communication skills.
  • Technologically savvy with a strong customer service ethic.
  • Exceptional organizational skills with the ability to manage multiple tasks effectively.

Preferred Qualifications

  • Background in health and human services preferred.

Physical Qualifications

  • While performing the duties of this job, the employee may be regularly required to stand, sit, talk, hear, reach, stoop, kneel, and use hands and fingers to operate a computer, telephone, and keyboard.
  • Specific vision abilities required by this job include close vision requirements due to computer work.
  • Light to moderate lifting may be required.

Compensation And Benefits

  • Starting salary is $23.00-$26.00 per hour depending upon skills and experience. We pride ourselves in maintaining a flexible and friendly work environment. We provide comprehensive benefits including: paid vacation (15 days); paid sick leave (80 hours); company paid medical, dental, vision, chiropractor/acupuncture, and life insurance; travel reimbursement; educational/fitness allowance; and pre-tax retirement plan with 6% of salary contributed by employer.
