Outreach and Engagement Coordinator/Case Manager

Responsibilities

  • Establish presence and build relationship within the community of San Bernardino.
  • Conduct initial and quarterly psychosocial assessments to develop and implement an individualized service plan, in collaboration with our program participants, based on the information gathered through the utilization of the psychosocial assessment
  • Maintain a calendar of outreach activities, including appearance, community events, and other communication opportunities.
  • Maintain positive relationship with potential clients, local community centers, and team members.
  • Prepare accurate records and reports on the goals of the services reach
  • Research potential location to do outreach and engagement with the San Bernardino County
  • Respond to client through engagement and serve as the primary point of contact between clients and program services
  • Work with Program Manager to strategize creative ways to spread awareness about the services offered.
  • Proactively contacts, connects and converts prospective clients to referrals
  • Conducts timely updates and ongoing reviews with Case Managers and Program Director
  • Demonstrates cultural competency and deep understanding of cultural sensitivity and special family and youth issues
  • Other duties as assigned.

Qualifications

  • Valid California Driver’s License, Auto Insurance and accept travel assignments on the job as directed by the Program Director
  • High School Diploma Required. Associate or Bachelor’s degree in social sciences, nonprofit management or equivalent experience preferred
  • Ideal candidate would have lived-experience with homelessness
  • Ability to effectively and persuasively represent the Good Seed CDC to a diverse client population
  • Strong communication and relationship management skills with ability to build and foster new relationships
  • Knowledge of landscape/hot spots within San Bernardino County.
  • Ability to be autonomous while still a team player, and committed to both transparency and accountability
  • Effectively works with varied personalities and situations requiring tact, judgment, and poise
  • Responds positively to changing demands and priorities
