Borinquen Medical Centers is based in Miami, Florida and is seeking to hire a full-time Outreach Specialist to join our team. This positions is responsible to inform the community and create awareness about the program; Responsible for coordinating community presentations to educate the community about our services and forming partnerships with BMC; transport patients as needed so they may participate in wellness groups and/or see their providers; Makes every possible effort to outreach high risk clients to be test.

To decrease transmission of HIV in the community, implementing a safe educational environment is required, by using this outreach specialist to link education and awareness with recruitment for testing services. The outreach specialist will participate with this team to educate in the community and perform testing/screening events and coordinate their linkage to care with the Health Navigator. The outreach specialist will promote community and organizational services in the community, using an unbiased approach to testing and to ensure proper continuation and expansion of HIV and PrEP education.

The outreach specialist will ensure that high risk clients are reached and will oversee providing partner services, counseling, and providing for partners at home test kits for HIV and STD.

The outreach specialist is in charge to track all home test kits provided to partners and follow up with compliance of testing and screening.

For those HIV negative clients, he/she will ensure patients to be linked in care for PrEP and retained in care all within our Rapid Access program. Responsible for recruitment of participants into testing and treatment programs. Makes every possible effort to outreach high risk clients to be tested and linked to care.

Tasks And Responsibilities

  • Conducts street and community outreach.
  • Recruits participants for testing, treatments and counseling services.
  • Engages street contacts in risk reduction discussions.
  • Gives out HIV/BHV/STD risk reduction information.
  • Monitors and follow-up on service delivery.
  • Educates newly diagnosed HIV persons in treatment and service options.
  • Makes home visits and reaches out to persons/patients that present an adherence problem to treatment.
  • Provides HIV pre and post counseling and testing.
  • Educates contact in the use of ORASURE.
  • Provides program with community referrals, providing name and location of available van and outreach sites.
  • Ensure data collection on a timely, accurate and consistent manner.
  • Distributes preventive materials including safe sex kits.

Requirements

  • High School Diploma, or equivalency and/or outreach experience.
  • Bilingual preferred.
  • HIV 104 Certification
  • HIV/AIDS 500-501 from DOH.
  • Strong interpersonal skills.
  • Ability to interact effectively with consumers, administration, faculty, and staff.
  • Handle the information with high level of confidentiality.
  • Considerable knowledge of functions of Social Service that involve working with, communicating with, and teaching people. These occupations often involve helping or providing service to others.
