Position Overview
As an RN Case Manager, you will collaborate closely with clinical teams to provide complex case management and strengthen the patient-primary care physician relationship. This remote position will require occasional travel (10%-15%) to clinical sites in Philadelphia and Harrisburg, PA.
What You'll Be Doing
Conduct comprehensive assessments of healthcare and psychosocial needs for patients.
Develop, implement, and maintain individualized care plans aligned with internal protocols and accreditation standards.
Provide evidence-based clinical interventions based on care plan goals.
Collaborate with patient navigators to manage non-clinical aspects of care plans (e.g., appointments, transportation, social determinants of health).
Monitor patient data continuously and adjust care plans as needed.
Develop patient-specific escalation plans with PCPs for acute exacerbations.
Educate patients on conditions and treatment options.
Advocate for patients and facilitate access to necessary services/resources.
Follow up on service or resource referrals and maintain updated documentation in EMR systems.
Utilize evidence-based clinical protocols and quality metrics.
What You'll Need To Have
Licensed Registered Nurse in good standing.
3+ years of RN Case Management experience in an outpatient setting.
CCM certification preferred; must obtain CCM within one year of employment.
Supervisory Experience Preferred.
Excellent written and oral communication skills to interact empathetically with patients, families, and interdisciplinary teams.
Strong computer skills, including proficiency in multiple EMR and Case Management systems.
Ability to meet accreditation and quality standards (e.g., NCQA, HEDIS).
