Registered Nurse Home Health/Hospice

As a Registered Nurse in Home Health & Hospice, you will play a vital role in providing high-quality care to patients in their homes or hospice settings.

In this position, you will be responsible for conducting comprehensive assessments, developing individualized care plans, administering medications, providing wound care, and monitoring patient progress. You will also collaborate with a multidisciplinary team to ensure coordinated and holistic care for patients and their families.

Requirements

To be considered for this role, applicants must meet the following requirements:

• Valid and active RN license
• Minimum of 1 year of experience in Home Health or Hospice care
• Excellent clinical assessment and critical thinking skills
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Compassionate and patient-centered care approach
• Valid driver's license and reliable transportation
