Rehab Department Outcomes Coordinator

  • DEFINITION/PRIMARY FUNCTION
    • The Rehab Outcome Coordinator is primarily responsible for the management of CMS IRF-PAI (Inpatient Rehab Facility Patient Assessment Instrument) and providing direct patient care. Other functions include compilation of data related to clinical outcomes/quality indicators, assisting in the preparation of accreditation process/surveys and involvement in special projects. The Outcome Coordinator will receive direction from the Inpatient Therapy Manager and Rehab Medical Director.
  • POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
    • Experience:
      • Minimum of five years experience in a rehabilitative setting.
    • Education and Training:
      • Minimum of Associate's Degree in a Rehabilitative Therapy field (PTA or COTA) required.
    • License, Certification & Registration:
      • MA COTA or PTA - Current MA COTA or PTA License in related field.
      • CPR - CPR is required.
    • Other Requirements:
      • Excellent communication (written and verbal) skills.
      • Excellent organizational skills.
      • Demonstrate ability to function efficiently with Word and Excel.
      • Ability to prioritize tasks and take direction from multiple sources.
      • Ability to be flexible in managing a patient caseload and the duties of Outcome Coordinator.
      • Must possess sight/hearing senses or uses prostheses that will enable these senses to function adequately so the requirements of the position can be fully met.
