The Rehab Outcome Coordinator is primarily responsible for the management of CMS IRF-PAI (Inpatient Rehab Facility Patient Assessment Instrument) and providing direct patient care. Other functions include compilation of data related to clinical outcomes/quality indicators, assisting in the preparation of accreditation process/surveys and involvement in special projects. The Outcome Coordinator will receive direction from the Inpatient Therapy Manager and Rehab Medical Director.
POSITION QUALIFICATIONS (Minimum qualifications are required unless stated otherwise.)
Experience:
Minimum of five years experience in a rehabilitative setting.
Education and Training:
Minimum of Associate's Degree in a Rehabilitative Therapy field (PTA or COTA) required.
License, Certification & Registration:
MA COTA or PTA - Current MA COTA or PTA License in related field.
CPR - CPR is required.
Other Requirements:
Excellent communication (written and verbal) skills.
Excellent organizational skills.
Demonstrate ability to function efficiently with Word and Excel.
Ability to prioritize tasks and take direction from multiple sources.
Ability to be flexible in managing a patient caseload and the duties of Outcome Coordinator.
Must possess sight/hearing senses or uses prostheses that will enable these senses to function adequately so the requirements of the position can be fully met.
