We are seeking a dedicated Remote Triage Nurse to join our team, committed to delivering compassionate, high-quality care to our communities. In this role, you will provide telephone triage services within the scope of a California Registered Nurse, adhering to established nursing protocols. You will play a crucial role as a clinical liaison, offering timely advice, education, and care interventions to address primary care needs effectively.
Key Responsibilities
Mission Support: Uphold our mission to provide excellent healthcare services to the most vulnerable in our community.
Effective Communication: Maintain clear and professional communication with patients, families, care teams, coworkers, and external agencies. Document information accurately in the Electronic Health Record (EHR).
Care Assessment: Determine the most appropriate level of care based on assessments and plan suitable interventions for patients of all age groups. Collaborate with other disciplines as necessary.
EHR Proficiency: Complete core training for the Electronic Health Record system and maintain proficiency. Use essential office technology software to communicate with the care team.
Clinical Resource: Serve as a clinical resource within the Call Center for immediate patient care assistance. Work with the Call Center Manager to identify gaps and suggest improvements.
Medication Management: Perform medication refills using established protocols. Offer appointments or communicate with care providers if patient refills fall outside of the protocol.
Emergency Follow-Up: Monitor patient Emergency Room visits and hospitalizations, scheduling necessary follow-up visits as per policy.
Care Coordination: Identify patient care gaps in coordination with the Quality Improvement Department and assist with patient care registries to enhance health maintenance activities.
Patient Outreach: Establish contact with patients to schedule overdue clinic visits or other types of clinical follow-ups as specified by a provider.
Team Collaboration: Facilitate teamwork within the Patient Centered Health Home team, and other departments, holding team members accountable for overall team excellence.
Task Management: Communicate frequently with the Call Center Supervisor and Director of Nursing to ensure tasks are completed promptly.

Requirements
Valid State of California Certificate of Registration as a Registered Nurse
Current BLS certification
Bilingual Skills Preferred
Knowledge of the use, administration, and effects of narcotics and medicines
Understanding of basic communicable diseases and infection control practices
Familiarity with California Child, Adolescent, and Adult Immunization Schedule
Proficiency in using Electronic Health Records (EHR)
