Position Overview

The Arc is seeking a full-time Wellness Healthcare Project Leader. Reporting to the Director of Nursing, you will oversee healthcare projects from start to finish. Working closely with employees, the people we support, and the larger community, the goal of this newly created role is to establish and maintain programs that result in overall better health (e.g. lowering obesity, managing/preventing heart disease/diabetes, proper nutrition, exercise etc.)

What You’ll Do

  • Program Design

Build collaborative partnerships with third party health and wellness providers, including but not limited to Federally Qualified Health Centers (FQHC), area hospitals, physicians and medical practices, pharmacies, and PACE providers to focus on coordination of care, sharing of care information, and co-creation of programs focused on the health and wellness of individuals.

  • Assessment and Data Collection

Work with our internal programs to complete initial and ongoing health screens and surveys aimed at identifying the supported persons’ physical, emotional, and mental health needs and to develop plans of care that are comprehensive in meeting those needs.

  • Wellness Planning

Plan, lead, and implement health-related group activities and events

  • Care Plan Communication

Participate in regular team meetings with necessary parties (nurse, program, and other staff) to review individual care plans

Requirements

What You’ll Need:

  • Two-Five years or more of experience in healthcare project management activities
  • Demonstrated ability in leading large project teams
  • Prefer experience working with adults with intellectual disabilities
  • Must have current Maryland Nursing License and CPR for health care professionals
  • Strong problem-solving, interpersonal, and leadership skills are required
  • Ability to maintain confidentiality a must
  • Flexible attitude
  • Strong oral and written communication skills, including the ability to effectively present information and respond to questions from internal/external sources
  • Proficient in Microsoft Office Suites (Outlook, Word, Excel, PowerPoint)
  • Self-starter with the proven ability to work with minimal supervision
