The recent incident at Royal Surrey and Ashford and St Peter’s hospitals concerning the "Surrey Safe Care" electronic patient record system is deeply concerning, but unfortunately, this is not an isolated case when it comes to EHR implementations. Earlier this year, the Department of Veteran Affairs confirmed that the Oracle Cerner EHR system caused six incidents of "catastrophic harm" to veterans, and four of those incidents led to the death of patients. (https://lnkd.in/ghfX9mAw) Their adoption of the Oracle Cerner electronic health record system, initially rolled out in Spokane, has been directly linked to four veteran deaths and several other instances of harm. This serves as a pressing reminder that while technological advancements can be transformative, their implementation, especially in sensitive areas like healthcare, requires meticulous planning, comprehensive training, and consistent monitoring to ensure patient safety and optimal outcomes. The unfortunate situations highlighted both at Royal Surrey and at the Department of Veterans Affairs underscore the immense risks associated with implementing complex IT systems without an ironclad focus on human factors. While technology offers promising advancements, its integration within healthcare is fraught with potential pitfalls that can have life-altering consequences. First and foremost, human factors and change management are paramount. When we talk about the "Surrey Safe Care" electronic patient record system or the Oracle Cerner system implemented by the VA, the negative outcomes aren't solely a result of technical glitches or system errors. They are a testament to the importance of understanding and managing how human beings interact with these systems. Poorly implemented technology can endanger lives. Such incidents underline the critical importance of not just creating advanced systems but ensuring they are seamlessly integrated into human workflows. While technical solutions can drive improvements, they cannot stand alone. They can inadvertently do more harm than good if not implemented with a keen awareness of their human impact. The tragic events reported are not merely a byproduct of flawed systems, but a wake-up call highlighting the indispensable need for a holistic approach where technology and human processes are seamlessly integrated.
