When paired with data like that from the #asrt (2022) which shows that the number of applicants to radiation therapy educational programs is at a two-decade low, healthcare systems must plan proactively across the whole staffing spectrum. This means adjusting some traditional role models, engaging with pre- or early-college students, and supporting education and training initiatives to meet the gap in the numbers of providers available vs the numbers needed to ensure accurate patient care. This came across my feed again today and I realized I was in a different mindset because I was struck by a different idea (practicing #ValueDiscovery skills). The #Hospitality industry knows well the value of "knowing the customer" in a way they could delight them with simple person-centered things. The fact that everyone is slightly different and it's "the little things" that matter. Think about the last time you checked into a hotel and compare that to the last time you had great hospitality (perhaps a resort, cruise, or vacation destination). No fancy pictures for this post, just a story. We had an administrator during a demo of Life Highlights tell us a story from just a few weeks ago. She talked about how a certain CNA was really good at learning about the resident's lives that she cared for. So much so that the CNA was considered a 'gold standard' when it came to senior care. We commented on how great it is to have employees who have the ability and skill to do this. The administrator then went on to say, "It's all good, except when the CNA leaves her shift, then all that knowledge goes with her." "This," as she pointed to the framed Life Highlights sitting in front of her, "would help that knowledge cross shifts and help all the staff care for our residents." “I had no staff to check in patients. I had no one to prep the room. I had no triage to cuff and weigh them and get the forms signed. I just had a flood of 4/hour backlogged to see me all pissed off. Charting all night. This every day. There was no training period -no real supervision of any kind.And I came straight out of school.” -NP I met last night. Her recent first job was at a well known urgent care clinic. She said all her classmates faced the same with this employer. She was no anomaly. Forget 🔻This is a 1955 coal mine -a toxic workplace in every way. The space is untended as are the customers and nurses well being. It’s hell,chaos. Not one happy person all day every day. Forget 🔻She was thrown to boiling pot with no mentor/training/advisor/doc/guideposts. See overwhelming volume was the mandate -oh,and prescribe at this location. Hundreds of clinics like these are closing because urgent care is transactional v relational in true #primarycare so “cross selling” meds,home infusion and DME isn’t going to happen. If they even had time to even discuss/refer/code it. Forget 🔻 There is no way to “do no harm” when totally green NPs see 30-40/day with literally no breaks. This is clearly dangerous and happening right now at a convenient clinic near you. Closing these for violating loose supervision regs isn’t going to happen.Lawsuits are rare. Hearings would be noise,soundbites and bluster with no action ty Bigs lobby. The press badly needs these advertisers. This has been going on for years despite the new talent wars for PCPs. But the model was DOA. Broken day 1 The answer always has and will be #directcare. She had never heard that 20-30,000 docs and nurses now have gigs in DPC with ✔️no coding ✔️no charting at night ✔️zero volume pressure -panels are 400-1000. ✔️She can work #nearsite/#onsite/#virtually so flexibility as a new mom is important. ✔️real time connection with asynchronous texting and calls when you’re available 🔸NPS scores rank 100% of these DPC who measure it as world class consumer experiences 🔸Employers are renewing at 98%+ 🔸Burnout evaporates. This is The Cure. The market and posts like these- just maybe -will prevail and eventually the press will tell this story. Academics will study it. Whitecoats will flee this evil,greedy dark age into the light as word gets out. I live in a future where everyone wins .

Sidelined: Sitting on the bench while your injured teammates struggle to play a team sport is painful. Healthcare right now, well at least from my seat on the bench? That’s what it looks like. In the wake of a once-in-a-century pandemic, many qualified physicians have experienced a gap in employment and clinical contact beyond their control. Family deaths, personal illness resulting from COVID-19, and the need to care for sick family members have left some physicians struggling to return to work. But do these circumstances erase their qualifications? As a solution, hospital institutions should consider creating a new fellowship to help these clinicians re-enter the workforce. This would be a far superior alternative to forced early retirement. After all, wouldn't the team rather have another clinician to help care for those who need a doctor? Unfortunately, these situations are all too common and often affect women who have children and are caring for sick older family members. But why wouldn't our system want all hands on deck, especially when there are qualified clinicians actually wanting to work? Let's not forget about these qualified physicians who are out of sight and out of mind. Controversial, but OPD allowance doesn’t make much of a difference if you’re only using it during emergencies. Because if you only use it during emergencies, how is an OPD cover different from an IPD cover? 😕 Waiting for symptoms to visit a doctor and getting tests when your medical condition worsens shouldn’t be the point of an OPD cover. An OPD cover should proactively keep you healthy through guided care and make the experience cashless while doing so.
