If a well designed and adequately funded clinical trial using ONLY trained ECPR teams, and stratified by time to effective ECPR and perhaps mechanism of disease, a definitive answer may be the result. Out of hospital CPR results are pretty dismal even with bystander and AED use, so any significant improvement in neurological recovery in suitable patient population defined by such a trial would be worth the investment. Sounds like a case for ARPA-H with some AHA/ACC/NGO funding.
