We're proud to announce a strategic partnership with Sentara Health to enhance our Thrive United 365 program. As the cost of healthcare continues to rise, quality care has become more and more difficult for our ALICE (Asset Limited, Income Constrained, Employed) population to afford. This collaboration underscores our commitment to addressing healthcare disparities in Northern Virginia and improving the quality of life for ALICE families. Sentara’s investment into Thrive United 365 will work to address the social determinants of health among vulnerable communities by providing holistic, place-based, care through trusted community partners. Place-based screening programs will be hosted quarterly, bringing a variety of providers to a convenient and commonly visited community location to provide screenings and preventative care services for chronic conditions most impacting the residents. Service provider referrals will include direct health services, or other essential needs to navigate their situations such as connecting food security programs, financial empowerment programs, or housing support. Join us in creating a healthier, more equitable community. 𝗣𝗔𝗥𝗧𝗡𝗘𝗥𝗦𝗛𝗜𝗣𝗦 | We believe that the most effective way to build healthy and resilient communities is through a community-based approach that fosters community participation, collaboration, and synergy among stakeholders to improve health outcomes, one community at a time. Aligned with these guiding principles, we created RAFI CARES, a program dedicated to building Alcantarahanon Communities with Access to Relevant and Equitable Primary Health Care Services. As part of our joint efforts to guarantee that the community's needs and gaps are addressed, we signed a Memorandum of Agreement with the Municipality of Alcantara. This partnership also aims to integrate and consolidate all existing health-related interventions, implement programs, and create responsive governance for healthy Alcantarahanon communities. #RAFInforms #SDG17 #PartnershipfortheGoals

Dear LinkedIn community, as we step into the second half of the year, I am filled with gratitude for the journey so far. Leading the SLUM AND RURAL HEALTH INITIATIVE NETWORK has been an incredible privilege, one that has allowed me to touch lives and bring about positive change in some of the most underserved communities. Reflecting on the past six months, I am thankful for the opportunities that have come our way. From groundbreaking health initiatives, to profound research undertakings, as well as impactful outreach programs, the work we do at the SLUM AND RURAL HEALTH INITIATIVE NETWORK continues to inspire and motivate me every day. It's heartening to see the difference we are making, but it's also a reminder of the immense work still ahead. And so to everyone reading this, remember that it’s never too late to achieve your goals. The second half of the year offers a fresh start, a renewed chance to strive for what matters most. Whether it's professional aspirations, personal milestones, or community impact, there's still time to make it happen. As we move forward, my commitment to healthcare advocacy remains unwavering. I am dedicated to expanding our reach, improving health outcomes, and fostering a healthier future for all. The support from our partners, funders, volunteers, and this incredible LinkedIn community has been invaluable, and I am excited about the continued collaboration and shared mission. So my dear friend, what are you thankful for in the first half of the year, and what are you committing to improve upon? Let's continue the conversation in the comment section. Here’s to a productive and impactful second half of the year. Together, let’s make every moment count. #healthcareadvocacy #SRHIN #goals #secondhalf2024 #communityimpact

Helping FQHCs, public health departments, and other health nonprofits win more grants, gain greater strategic clarity, and meet community needs. Writer. Facilitator. She/her.
Health centers, here is a cool and important opportunity from Vot-ER! It's not huge dollars, but if you work in Black, Brown, Native, and low-income communities and are planning to implement #voterengagement initiatives, you should consider applying for their 2024 Community Civic Engagement Program. It provides hands-on support and small #grants ($5,000 to $10,000) to health centers and look-alikes for implementing voter engagement initiatives using the existing operations of their #communityhealthcenters. There's an informational webinar about it TODAY and applications are due March 11th. Learn more below, or I'll put a few links in the comments. 👇🏽
We had the pleasure of speaking at the National Association of Community Health Centers (NACHC) conference held in Washington DC during the “Improving Health Outcomes through Civic Engagement” panel alongside some other stellar panelists including Sherry Williams, MPA of Health Point CHC, Sarah Francois of NACHC, Robin Boyles of Delta Health Center Inc. Health centers can apply for the 2024 Community Civic Engagement Program to receive hands-on support and a small grant to implement voter engagement initiatives: vot-er.org/ccep Through our grants, community health centers can engage patients in voting for the betterment of their centers.
