Patients presenting with gastrointestinal signs can pose a challenging workup because the pathology of these disorders can be multi-factorial. Diseases such as IBD, pancreatitis, PLE, LSA, SIBO, Addison’s, and allergies may exist concurrently, so multiple tests, imaging, and other workups are needed to guide the differential diagnosis. A comprehensive analysis of GI disease facilitates a quicker diagnosis, reduces tech time and hospital visits, and limits resource use. vtx:think ⁠ ⁠ The following patient presented because the owner noticed bleeding from the mouth. On examination, you notice multiple petechiae all over the inside of the gums. ⁠ ⁠ Petechiae and ecchymoses are red to purple discolourations of the skin or mucosal surfaces, resulting from small blood vessel bleeding. ⁠ ⁠ Petechiae are typically mm in diameter, resulting from capillary bleeding; ecchymoses are larger lesions caused by bleeding from small arterioles and venules. ⁠ ⁠ Signs of surface bleeding usually result from a platelet or blood vessel abnormality but can be due to a more global bleeding disorder.⁠ ⁠ Blood vessel walls are lined by a single layer of endothelial cells, connected by tight junctions and anchored to a basal membrane. This selective barrier prevents the leakage of red blood cells from the circulation. ⁠ ⁠ The blood vessel has an active role in haemostasis. With vascular injury, the subendothelial matrix is exposed, allowing platelets to interact with collagen and other matrix molecules. ⁠ ⁠ Circulating platelets adhere to the site of the vascular defect, and recruit even more platelets to aggregate. This demonstrates the essential role of platelet-vascular interactions in maintaining haemostasis. ⁠ ⁠ Thrombocytopenia is the most common cause of petechiae and ecchymoses; thrombopathies and vascular diseases can also cause signs of surface bleeding.⁠ ⁠ What is the best way to check platelet numbers in this patient?⁠ What are the most common causes of thrombocytopenia?⁠ Do ECC cases fill you with dread? vtx:think In-house coagulation parameters Prothrombin Time (PT) and Activated Partial Thromboplastin Time (APTT) are becoming more widely available, but what constitutes a significant result and what should we do about it?!   Bernie is a 6yo male entire Shih Tzu who lives on a farm with his 12yo brother Jinx. Jinx was sadly euthanised three days ago when he collapsed having become lethargic with laboured breathing. Bernie has presented today with similar clinical signs today and the owner has feared the worst. Whilst collecting the clinical history you ask the owner about access to toxins or human medications. The owner noted the farm Bernie stays on has a persistent mouse problem, but Bernie is a sensible dog and whilst he has free roam of the farm, he wouldn’t eat any mice.   We were concerned Bernie may have been exposed to rodenticides during the last week and decide to check his PT and APTT.   🤔 How do you interpret the results? 🤔 Are they significant? References ranges shown below for both parameters: PT (14 – 20s) APTT (94 – 123s)   Bernie’s coagulation profile is severely abnormal because both values are more than 30% above the top of the reference range. This marked prolongation of PT is consistent with rodenticide toxicity. You recall plasma contains clotting factors but can’t remember which product is most appropriate. Which clotting factors do you need to replace and why? His coagulation parameters were repeated in the morning following his 20ml/kg fresh frozen plasma transfusion. Bernie was continued on his vitamin K treatment and made a full recovery. Do ECC cases fill you with dread? Ongoing research is focusing on feline respiratory syndromes comprising "asthmatic" (eosinophilic) or "bronchitic" (neutrophilic) airway inflammation. However, this classification has its limitations, primarily because it relies on collecting airway fluid through BAL, and it doesn't encompass the intriguing cases of mixed airway inflammation. November is #PetCancerAwarenessMonth. According to the American Veterinary Medical Association (AVMA), 50% of dogs over age 10, and 25% of all dogs will develop cancer. Although there is less information for cats, our feline friends also commonly develop cancer. Cancer can develop in any body system. Symptoms vary according to the organs affected, but may include: • A lump or mass anywhere on your pet's body • Difficulty eating or chewing, or a decreased appetite • Weight loss • Persistent vomiting, drooling, or diarrhea • Difficulty breathing • Bleeding from the nose or mouth • Abdominal swelling • A wound that will not heal • Limping or lameness • Excessive drinking/urinating • Straining to defecate or urinate Other diseases may cause similar symptoms, but reaching a diagnosis as quickly as possible is important. vtx:think Disorders of cobalamin (vitamin B12) metabolism are more commonly identified in small animal medicine and have a variety of causes ranging from chronic gastrointestinal disease to hereditary defects in cobalamin metabolism. Measurement of serum cobalamin concentration (often in combination with serum folate concentration) is more often being performed as a diagnostic test in clinical practice. While the detection of hypocobalaminemia has therapeutic implications, interpretation of cobalamin status in dogs can be challenging.    Cobalamin should be supplemented whenever serum cobalamin concentration is below normal levels (less than approximately 400 ng/L). Cyanocobalamin has commonly been used for supplementation, as it is widely available and  inexpensive.  In dogs and cats with suboptimal cobalamin status, cobalamin is recommended to be given at a dose of 50μg/kg SC every 7 days (for 6 weeks) or 50μg/kg PO q24h (for 12 weeks).   Question… Is there a benefit to supplementing orally over injectable forms?
