"A cluster-randomized controlled trial of a nurse-led artificial intelligence assisted prevention and management for delirium (AI-AntiDelirium) on delirium in intensive care unit: Study protocol." Plos one 19, no. 2 (2024): e0298793. Summary: Background: - Delirium is common in ICU patients and worsens their health outcomes. - Following the PADIS guidelines ("ABCDEF bundle") is important for preventing delirium, but adherence is low in practice. - AI-AntiDelirium, an AI-assisted tool, was developed to help nurses prevent and manage delirium. - A pilot study showed positive results for using AI-AntiDelirium in preventing delirium and improving patient outcomes. Methods: - A large-scale clinical trial will assess the impact of AI-AntiDelirium on ICU delirium. - Six ICUs in two hospitals will be randomly assigned to use AI-AntiDelirium or follow the PADIS guidelines. - Over 1450 patients aged 50+ will be included from each group. - The study will measure various outcomes, including: Incidence of ICU delirium Duration of delirium Length of ICU and hospital stay Mortality rates in ICU and hospital Patient cognitive function and daily living activities Nurse adherence to the ABCDEF bundle Discussion: - This trial aims to evaluate if AI-AntiDelirium effectively reduces delirium and improves outcomes while increasing adherence to guidelines. - If successful, it could significantly improve ICU delirium management in research and practice. I am interested to see the results of this one.

A Monitoring Visit in Brief: the 6 steps! 🏃♀️💨 Not long ago we went through an "SIV: The elevator pitch". It's time for the Monitoring visit. 🙌 Sweet! Step 1👇 Arrive at site, meet with your investigator or study coordinator and settle in the monitoring space (it can be a room dedicated to Clinical Research Associates (CRAs) but it can also be your study coordinator or investigator's office. 🏥 Step 2👇 Go straight to monitoring. Meaning, get your checklist out for this visit and start work. Review the action items pending, monitor the queries open or responded, monitor any reconsents or newly enrolled patients, continue with monitoring the new data generated since your previous visit and of course, do not forget to monitor the Investigator Site File (ISF). ✅ Step 3👇 Remember to note down (in whichever app you use to take your Notes - I use Notion) e-very-thing, every small thing you monitor. It's the only way you will remember to report and just in case, in 2-3 weeks, your sponsors asks you something related to this visit, you have it in your notes. 😮💨 Step 4👇 Scan those documents you are missing in the eTMF. If you are allowed to use a Scan app, do so (I did, I used CamScan) but if you're not because your SOPs forbit it, take a photo, use your site's scanner (if they have) or have a portable scanner with you (ugh, such an annoying little device😒). Step 5👇 Keep all your documents very (!) organized. If you bring any printed copies from home, have a folder specifically for this study and this site to avoid messing up files from this site with others.
