🧠❗Excited to share our most recent research publication in the journal Dermatitis! https://lnkd.in/dAqw3r4u ❣This was a special one for me as not only was it the first project I worked on when I began in my role at the National Eczema Association, but it allowed me to bring my interest and expertise in the field of mental health, and apply it to our goal of understanding the patient reported burden, and associations, between mental health and atopic dermatitis.
Today, the National Eczema Association published new research on eczema and mental health in the journal Dermatitis. Based on a 2022 survey of eczema patients and caregivers, the study reveals that adults, children and teens with eczema experience a significant mental health burden that increases with eczema severity. Interesting case on MSK-related neurogenic dysphagia. A 28 year-old male patient contacted me for severe difficulty swallowing. It had been getting gradually worse, starting off as something that would cause intermittent difficulties with swallowing solids, but unfortunately with increasing levels. At the point of consultation, he was having difficulty swallowing liquids as well, even saliva. The patient admitted that he had some very stressful months prior to onset of symptoms, but was still in full employment and denied any history of full blown psychiatric disease. He said that the problem tended to be better in the morning and worse in the evening. Worse with stress. Worse when working at the computer. Major concerns in cases like this is insidious onset of autoimmune disease, especially MS. He only had a cervical MRI donne, which was clearly not adequate. Also, ENT visits had not resulted in any clear findings. I requested a brain MRI + blood work for inflammatory processes, which were all normal. Thankfully. In strange syndromes of musculoskeletal origin, it is crucial to look at factors of worsening and amelioration, especially worsening, in order to figure out the mechanism of induction of symptoms. Without a known mechanism it is almost impossible to solve the mystery. He did say that computer work made it worse. I asked him if arm-loading seemed to trigger it, as lumpy throat is sometimes seen in thoracic outlet syndrome, but he didn't think so. I did the Eden's test and various others, but improvement nor worsening was seen, at least at a glance. I told him that this could be jugular outlet syndrome, and that the dysphagia could be secondary to glossopharyngeal or vagal nerve impingement. I requested a CT scan of the craniovertebral junction to look at the interval between the c1 lateral mass and styloid processes. I also had him do a cervical tucking test, which will draw these bones further together. I had him hold the position for a couple of minutes. This could go three ways: No change, better, or worse. After holding the position, I had him attempt drinking again, which was now much easier. This suggests a couple of things: 1., the problem is most likely coming from this area. 2., the procedure numbed the nerve, which causes intermittent improvement usually followed by brutal worsening. This is what happened. I had him start conservative rehabilitation for jugular outlet syndrome, which is more or less upper cervical rehabilitation. This was a couple of months ago. I saw him again a few days ago, where he had seen marked improvement in his condition. He no longer had difficulty with fluids, and only sometimes struggled with solids. But his symptoms would exacerbate when he strayed away from the protocol that he had been placed on. I also evaluated the CT that was ready. It showed some narrowing, but not severe. In other words, the stenosis is mainly postural. This has much better conservative progn
