Let's talk about shame in our healthcare corridors. Why SHAME? Each one of us has either been shamed or shamed someone in the line of care. Sometimes it is patients, most of the time it is our colleagues. And we do it both consciously and subconsciously. The shame culture in healthcare is woven thickly into the fabric of our profession. We are taught to associate vulnerability with weakness, instead of courage. And to never show any weakness. In healthcare, each day is filled with uncertainty, risk, emotional exposure and new discoveries. The way we handle shame impacts the way we treat each other and communicate with patients. It affects our responses to medical errors and affects the way we relate with each other at work. We are trained to recognize discomfort in patients. We are not trained how to recognize our own pain or name the emotions we feel. We are not trained to cope effectively with death, medical errors or malpractice suits. These very issues are what perpetuate a toxic culture leading to burnout, high turnover and very poor patient experience. And the environment we find ourselves in becomes the greatest enabler of the shame culture. Learning to deal with shame is part of self-care in health care. And when clinicians practice self-care, patients receive exceptional care. Physician burnout has been in the limelight, but the narrative around it doesn't capture the entire story. There's a deeper, often overlooked, dimension known as moral injury. While burnout hints at individual fatigue, moral injury focuses on systemic failures preventing doctors from prioritizing patient care. Most interventions have focused on individual resilience, insinuating that the individual is somehow deficient & needs "fixing.” We need inclusive & system-wide conversations about what healthcare workers actually need to do their jobs safely, ethically & with meaning.

Did you know that physicians have one of the highest suicide rates of any profession, with an estimated 119 physicians dying by suicide every year in the United States? It is essential that as leaders in healthcare, we talk about and address moral injury. As Dr. William A. Haseltine explains, “The term is defined as the strong cognitive and emotional response that can occur following events that violate a person's moral or ethical code or when they feel betrayed by authority figures. It should be distinguished from the term "burnout," which is often used to describe the effects of ongoing stress in the workplace, but the two conditions can overlap.” In the healthcare environment, we must remember that moral injury can be caused by: ✔️ Witnessing chronic patient suffering ✔️ Experiencing staffing shortages ✔️ Enduring abusive or poor leadership ✔️ Feeling conflicted over prioritizing patient care over their own personal safety ✔️ Not having the chance to debrief after experiencing tough patient cases or situations With these alarming statistics getting worse each and every day for all of our healthcare clinicians and professionals, we must take steps immediately to address the culture, reduce any administrative burdens, and create a healthcare system that is truly people-focused. We have to be willing to acknowledge and face these problems head on. They aren't easy to talk about, especially when they affect you personally. At the core, being a physician is a noble profession. Even if it is just another day at the office for them, physicians are having more impact on their patients than they would ever realize. The problem is that patients have 15 minutes with their doctor and little ability to acknowledge their work. When the "bosses" do the opposite of acknowledgement, it has to be crushing on their souls. You want to feel good about your work, but you are 'rewarded' by longer work hours, more patients in less time, and a tighter grip from insurance and big business. The problem hides in plain sight, the statistics show the end results of the problem, yet the cause of the problem keeps pushing forward at a faster pace.
