CVS Health is entering the business of selling private label biosimilar products with significant repercussions for biosimilar manufacturers. Last year, CVS Health launched Cordavis in an effort to partner with manufacturers to commercialize biosimilar drugs in the backdrop of several long-awaited Humira biosimilar launches. As its first partnership, Cordavis collaborated with Sandoz to commercialize Hyrimoz under a Cordavis private label. In addition, on April 2024, CVS Health removed Humira from its Advanced Control Specialty Formulary and added Hyrimoz as the preferred adalimumab product. The preferred options in the CVS Health Advanced control Specialty Formulary for 2024 are the high-WAC Hyrimoz from Sandoz, the low-WAC private label Hyrimoz from Cordavis, and the low-WAC unbranded adalimumab-adaz from Sandoz. The approach is driving a significant increase in new patient prescriptions for Hyremoz although Humira still retains the lion’s share of the market for existing patients. The collaboration with Sandoz is likely the first of several efforts by CVS Health and other PBMs to penetrate the biosimilar market. In the coming years, several high-profile specialty brands will face LOE as key patent expirations materialize. These specialty brands represents a massive opportunity for CVS Health and other PBMs in a biosimilar market that is projected to grow to over $100B by 2029. By establishing Cordavis, CVS sets the stage to compete directly with biosimilar and innovator products in the very marketplace it manages. The move grants CVS Health several advantages, including: - Higher margins: Store brands generally offer higher margins. In this case, it provides CVS the opportunity to share the biosimilar manufacturer’s profit margin - Greater negotiating power: A private label biosimilar provides CVS additional negotiating leverage with manufacturers of biosimilar and innovator products by strengthening CVS Health’s ability to access the product and secure favorable pricing - Patient loyalty and brand management: The move drives customer loyalty and builds the CVS Health brand while simultaneously promoting Sandoz’s product through the comparatively more established CVS brand Biosimilar manufacturers will need to recalibrate their launch strategy to anticipate direct competition from PBMs marketing private label biosimilar products. Manufacturers can begin to navigate these dynamics by assessing the risk of white label competition from PBMs, developing strategic partnerships with PBMs and other stakeholders, getting crystal clear on their value proposition to payers.

Dual brand pharmaceuticals - gaming or fair game? Last week Charles River Associates released a brief report on the ins and outs of dual brand pharmaceuticals: Active ingredients registered and sold under different brands for different populations/indications. This is different from multi-indication use, classically such as adalimumab initially approved for rheumatoid arthritis and subsequently expanded to treat many other conditions, but with no variation to the product or brand supplied. A topical example is semaglutide, marketed as Ozempic for type 2 diabetes, and Wegovy for weight loss. The molecule is delivered in self-administered prefilled pens but with different dosages, reflecting the different therapeutic profiles needed, and different prices. Proponents argue dual branding is appropriate when there are substantial differences in patient populations, unmet need and value to patients/payers, where additionally product characteristics (dose, route of administration, formulation, site of care, role in regimen, etc. ) are different. In their report, CRA finds that payers support dual branding when there is "a strong rationale beyond just a difference in indication, which drives the differences in dosing, formulation, or ROA [route of administration], to justify a dual-brand approach". This is in contrast to simple dosage variations. When there are concrete and quantifiable differences in therapeutic profile, realised by studying additional patient groups, dual branding appears to be an appropriate commercial strategy supported by payers. Pfizer has just done something incredible - it’s now selling some of its medicines direct-to-consumer. This is long overdue in the pharma industry. In many ways, this is excellent news! The news is part of a larger industry push to cut out middlemen and simplify the distribution of products. This will cut costs significantly and make it much easier for people to get the medication they need without jumping through hoops. Pfizer’s website will connect patients with consultants to prescribe the drugs and then a supplier who will fill and ship the prescriptions. The plan is to sell products like its COVID treatment, Paxlovid, and its migraine nasal spray. If you want to innovate, you need to do things differently, and I think this is both a bold and clever move by Pfizer.

Therapeutic Goods Administration have updated their guidance on advertising cosmetic injectables to ensure advertising rules are applied consistently across all industries that deal with therapeutic goods. Recently, the TGA has observed that clinics and health services in other industry areas are advertising the availability of prescription medicines, usually by referring to a class of goods (for example ‘weight loss injections’, ‘medicinal cannabis’ or ‘nicotine vaping products’). The TGA has interpreted that promoting a health service in this way is also an advertisement for a therapeutic good that refers to prescription medicines, which is unlawful. To resolve any inconsistency in interpretation across industry areas, the TGA no longer permits references to terms such as ‘wrinkle reducing injections’ where those terms would result in a reasonable consumer understanding the intention of the content is to promote the use or supply of a prescription medicine. The revised guidance on advertising #cosmetic injectables is now available on the TGA website. This material reinforces that the legislation itself regarding cosmetic injectables has not changed, and advertising the use or supply of prescription medicines, including most cosmetic injectable products (as opposed to services), has never been compliant with the therapeutic goods framework.
