Hi, growing interest in school-based integrated care (SBIC) as a solution to inequitable access to healthcare for school-aged children in urban and rural settings is supported by evidence of positive outcomes for young people and provision of acceptable "one-stop shops" for accessing care. Please see our latest publication describing how SBIC can provide child and family care. https://lnkd.in/g7s5b3_2

I hope that, this achieves all we hope it will. A thought provoking session, the day after the funding debate in Parliament. 🤔 nearly at the end of month 1 of the 2024/25 financial year, how quickly time passes!! #Hospices #equitablefunding

#AdvanceCarePlanning is becoming more and more important. The right to choose the setting, to prepare oneself and family members, to fully live each day is a meaningful issue. Even more important in case of children, whose family need to be prepared and to plan ahead how, when and where. 🎯Register to the EAPC free webinar 🗓️13th October 2023 #PalliativeCare #PalliativeMedicine #PalliativeCareForChildren

Camden Coalition has a bit of a cult-like status within complex care models, and the work there is ground-breaking, mission-oriented and thoughtful. Given my admiration for them, the studies showing their lack of demonstrable results point to a simple fact: Care coordination alone, even when implemented well, is insufficient to reduce hospitalizations for this [really, any] complex population of high-need patients. Primary care has proven its value. I am more concerned that a lot of the innovative, Specialty-focused models seem to rely on navigation, coordination and peer supports - without learning the lessons that are tried and true about complex populations. The reality is that these populations actually need a place to go when their needs escalate, a primary care provider to help them and real specialist access/integration for their needs. Let's hope that the next wave of these innovative companies learn these old lessons - these populations deserve it. https://lnkd.in/ecsjcbY2

“Today, one in four Ontarians with ALS choose medical assistance in dying (MAID), and increasingly, they are citing poor support as the reason. We can do better. We must do better.” – Tammy Pighin Moore, ICD.D, CEO, ALS Society of Canada Ontario’s health framework isn’t designed to address the unique challenges of ALS – a devastating disease that continues to take and take. In today’s Toronto Star, we look to raise awareness of the current realities of the province’s health care system, which fails to meet the complex and urgent needs of people living with ALS, leaving more than 1,300 Ontarians without adequate care and support. This op-ed comes at an integral time as ALS Canada will come together with the ALS community tomorrow, February 21, for a Day of Action at Queen’s Park. ALS Canada urges the Ontario government to invest $6.6 million of crucial funding in Budget 2024 to support Ontarians living with ALS and their families with their health care needs as part of the Ontario Provincial ALS Program. Some live key takeaways from our webinar with Caitlin Synovec from National Health Care for the Homeless Council: ✔ Hospitals can provide consistent, predictable funding for programs through contracts and rates. ✔ Even in states where other funding mechanisms exist or are in development, hospital funding supports operational costs and non-billable services. ✔ Medical respite can contribute to hospital goals such as decreased length of stay and reduce readmission rates.

One of the essential benefits of partnerships between hospitals and community-based organizations is the potential for more efficient use of resources. When organizations collaborate, they can pool their resources and expertise to build stronger neighborhoods and provide better care for the communities they serve. Let’s listen to and include our community members; they are context experts having a unique understanding of the world around them. #community #socialdeterminantsofhealth #healthcare
