I agree with Professor Mintzberg's thoughts in this paper, especially regarding the disconnect between managers and healthcare professionals (clinicians). Indeed, my doctoral thesis examined the disconnect between governance, management, and operations in healthcare, and found the Viable System Model (VSM) to be a very applicable and useful model for understanding these disconnects and how to address them. In terms of efficiency - unfortunately cost-cutting is often seen as the way to make a system efficient but all it really does is make it cheaper in the short term, at best. Efficiency, that is to apply resources to best effect without waste, has to be designed into a system. As health organisations are generally large, complex, and face rapid change, model-based-management is not only useful, it is necessary. The VSM is an excellent model for setting organisations up to distribute the management of complexity so that decisions are made close to their source. Its two-way communication channels set up the conditions for achieving 'respectful collaboration' and understanding between governance, management, and operations throughout the organisation.
