NC Department of Health and Human Services’ Post

The White House and the Centers for Medicare & Medicaid Services (CMS) have demonstrated their prioritization of the crucial task of addressing health equity through health-related social needs. These social factors, such as housing, and access to nutritious food, play a significant role in shaping individuals' health and well-being. By focusing on these factors, we can make a positive impact on improving health outcomes for all. Here in North Carolina, the Healthy Opportunities Pilots (HOP) program is doing just that. Through HOP, NC Medicaid has implemented 28 CMS-approved services aimed at addressing food insecurity, housing instability, transportation insecurity and interpersonal safety and toxic stress. Since March 2022, more than 420,000 services have been provided, helping to address housing move-in support, transportation vouchers, and healthy food boxes (and more) to over 24,000 Medicaid members (as of May 31). By addressing the social determinants of health, NCDHHS is working to have a lasting positive impact on the lives of many more North Carolinians and improve health outcomes across the state. #Healthcare #SocialDriversOfHealth #Equity #CMS #WhiteHouse #HealthcareInnovation #CommunityHealth Read more: https://lnkd.in/ehjAJGsr
