61 year old hypertensive and diabetic apparently normal developed vertigo and vomittings for few days, treated at a local hospital and MRI brain revealed right medullary ischemic infarct. Managed conservatively. After few days he developed numbness of right upper limb. Arterial and venous Doppler revealed multiple thrombi in the brachial artery, axillary artery and ulnar artery. He was treated with anticoagulants in the second hospital. On further evaluation, Coronary angiogram was done which revealed triple vessel disease with Left main coronary artery occlusion requiring cardiac surgery and referred to tertiary care hospital. Throughout the course, the root cause for these clots was missed and undiagnosed despite the guidelines for 7 days of ambulatory ECG monitoring mandated for all ischemic CVA patients. Patient travelled across 3 Hospitals and 2 states, getting fragmented care without a definitive care. This signifies the need for care continuum and patient centric care excellence rather than speciality centric care.
