Transcatheter aortic valve replacement (TAVR) can be performed safely in adults with congenital heart disease (CHD) if done by a multidisciplinary team of specialists at a high-volume center, so concludes a Cleveland Clinic case series. TAVR can be an effective intervention in CHD in select clinical scenarios, proving both feasible & safe with good short-term outcomes. In high surgical risk, it can be used as a bridge to recovery, future surgery, transplantation, or palliation. Joanna Ghobrial Tara Karamlou Grant Reed https://lnkd.in/gMXUfMdj

Dr. Serdar Farhan discusses the implications of his study on the underrepresentation of women in clinical trials for endovascular therapy and bypass surgery for symptomatic femoropopliteal peripheral artery disease. "Due to the lack of sufficient clinical evidence specific to female patients, interventionalists face challenges in determining the most effective treatment approach for women," says Dr. Serdar Farhan #MedicalResearch #Cardiology #EndovascularTherapy #PeripheralArteryDisease #ClinicalTrials #WomenInMedicine https://lnkd.in/egYCSTew

New #EJVESOA "Perioperative Mortality and Survival after Repair of Abdominal Aortic Aneurysm in Advanced-age Patients: A National Study from the Norwegian Registry for Vascular Surgery Focused on Nonagenarians," by Kristin K. Vikan, arne seternes, Linn Hege Nilsen, Erik Mulder Pettersen and Martin Altreuther #aortaed https://lnkd.in/dRe6fDHK → This paper from Norway identified 77 >90 years old, 1 362 80 - 89 years, and 4 590 <80 years operated for AAA 2015-21. In asymptomatic nonagenarians, 30D mortality rate was 2.5%, with good 3 year survival. Median survival after rupture was only 0.1 year. What is your decision algorithm for nonagenarian patients with AAAs⁉️

Post-Operative Atrial Fibrillation (POAF) is a common and costly complication that occurs in 20-55% of cardiac surgery patients and has increased risk for stroke, thromboembolism, and mortality. Risk factors and etiologies for POAF are poorly understood. Patients who develop POAF have a two-fold increased risk in all-cause 30-day and 6-month mortality and spend 3.7 more days in the hospital on average than patients who do not develop POAF (increasing the cost of a patient’s hospital stay by about $15,593). Lopes, L. A., & Agrawal, D. K. (2022).

This study explores the role of the Systemic Immune-Inflammation Index (SII), derived from platelet, neutrophil, and lymphocyte counts, as a predictor of in-hospital mortality for patients undergoing on-pump cardiac surgery. Analyzing data from 480 patients, it was found that a higher preoperative SII is independently associated with an increased risk of mortality. #CardiacSurgery #InHospitalMortality #OnPump #PrognosticTool #SystemicImmuneInflammationIndex

Recent publication about carotid interventions and outcomes… ✅ the main objective of this study is to evaluate and compare the outcomes regarding operative mortality rate, ipsilateral stroke, primary patency and overall survival rate among patients with internal carotid artery (Ica) stenosis submitted to carotid endarterectomy or stenting. ✅The perioperative mortality rate was 1.7% in total cohort (two patients), with no differences among endovascular and open surgery groups (0% versus 3.1%, P=0.31, respectively). furthermore, the incidence of post-operative stroke was 2.6% (asymptomatic 1.7%, symptomatic 2.9%), with no differences among endovascular and open surgery groups (2.0% versus 3.1%, P=0.42, respectively). ✅a univariate and multivariate linear regression analysis was performed in order to analyze the factors related to post-operative stroke. among the factors evaluated, only cardiac disease was related with Hr=2.1 (P=0.037, cI 1.02-2.115). a univariate and multivariate linear regression was performed to test the factors related to perioperative mortality rate. among the factors evaluated, only female gender was related with Hr=2.37 (P=0.019, cI 1.26-2.66). ✅Independently of the type of carotid intervention (cEa and cas), both techniques are safe and secure, having similar results regarding postoperative outcomes for stroke and death. female gender was associated to increased risk of death while coronary artery disease history was associated to increased risk of perioperative stroke. #vasculariamspe #endovasculartherapy #carotidartery #carotiddisease #stroke #vascularsurgery
