Social Factors Linked To Health Disparities In Older Adults Social determinants of health (SDOH) factors were associated with reduced access to therapy services. Examples of SDOH include: 1. Dual Medicare and Medicaid eligibility 2. Rural residence 3. limited English proficiency Older adults who were eligible for both Medicare and Medicaid had a 30% lower chance of receiving any PT or OT during their hospital stay compared to those with only Medicare coverage. Similarly, older adults residing in rural areas had a 35% lower likelihood of receiving these services compared to their urban counterparts. Timely access to mental health services is a vital part of patient-centered care and for many #raredisease patients, directly correlated with key health outcomes. Unfortunately, the fragmented U.S. healthcare system often makes it difficult for patients to access the mental health services they need, and the problem is particularly acute for patients from historically undeserved communities. The new 8-year Centers for Medicare & Medicaid Services Innovation in Behavioral Health (IBH) Model is trying to change this, promising meaningful progress for patients and families around the country (or at least the 8 states in which the program will be piloted). 2wMedicaid is a major cause 1. of unmet crises in behavioral health, 2. of treatment failure in the management of chronic disease 3. of excess deaths 4. of access barriers specific to those with Medicaid as providers avoid Medicaid or say that they take Medicaid when they do not 5. of access barriers where other worst public and private plans 6. of the closures of hundreds of hospitals and thousands of practices where there are concentrations of the most vulnerable populations 7. of health care decline with related economic decline and losses of jobs and local leadership in health and other areas
