I am sure Florida is not the only state with a shortage of Primary Care Physicians, but it is very difficult to get them to address the physical needs of our Clients, let alone the mental health issues because you have to spend extra time and attention to, to “peel back the onion” of mental health issues. Our PCPS are understaffed and expected to see many more patients now. As a nurse and patient advocate who works with many elderly in our communities, I find that we are bringing the mental health concerns to the physicians, and advocating for diagnosis and proper treatment. Dementia is often the diagnosis, but I am finding many of our Clients have undiagnosed mental issues. These are the ones getting “kicked out” of many nursing homes for aggressive or abusive behavior and we get the call to help advocate for a family who are totally frustrated and worried about their loved ones and don’t know what to do. Primary care providers are uniquely positioned to play a vital role in overcoming these barriers and ensuring that older adults receive the mental health care they need. • Primary care providers can screen for mental health conditions, provide brief interventions, and make appropriate referrals. • Collaboration between primary care and mental health specialists is essential for providing comprehensive care. • Ageism and stigma can prevent older adults from seeking help for mental health issues. • Primary care providers can help to reduce stigma and create a welcoming environment for older adults to discuss their mental health concerns. 2wAdvanced primary care is the way, but CMS is regressing primary care. Witness the Center for innovation failures, the harms done by Medicare Advantage, and by the abuses of the 3% cut in Medicaid for the SDoH waiver Over and over CMS targets delivery team members with more multitasking and designs that shape even fewer and even less suitable delivery team members plus higher turnover and burnout. Representative Doggett covered it well in this post about CMMI and Medicare Advantage. The part about discrimination against those most behind could be fleshed out more since the impacts are specific to providers that care for populations inherently lower in outcomes and drivers of outcomes is important to understand. The purpose of health policy should be to support those delivering the care, especially for the poor and the elderly.
