Backlogs in hospitals because SNFs aren't staffed well enough to take admissions piled on top of the looming marriage of observation and admission hospital stays that will give folks better access to care. Nursing home residents confront severe consequences from understaffing, particularly worsened by the COVID-19 crisis. President Biden's reform agenda, announced in 2022, tackles numerous pressing issues in nursing homes, including enforcing care standards, enhancing transparency, and curbing the misuse of medications. However, while the proposed nurse staffing ratios are a step forward, they fall short of addressing the scope of the problem. Concerns linger about the adequacy of proposed staffing levels, the effectiveness of enforcement mechanisms, and the delay in implementing new standards for rural facilities. Industry opposition, citing potential closures, faces scrutiny, as evidence suggests other factors contribute to facility shutdowns. CMS must heed calls for stronger staffing standards in the final rule to ensure residents receive the care they deserve, especially as nursing homes recover from the pandemic's impact. 🚨 I’ve seen a lot of chatter regarding this and I think a lot of folks are missing the point. Some things to remember from the study: 1️⃣ Authors studied ratios lower (3.88 HPRD) than what’s been deemed optimal - 4.1 HPRD 2️⃣ Outcomes got BETTER as staffing increased 3️⃣ It’s possible they didn’t find a minimum because the minimum should be at or higher than 4.1 4️⃣ Residents are coming to us sicker and more complex than ever before. The 4.1 number would have had RUG-IV used as an adjustment (I think) which doesn’t adequately showcase acuity. I’m not a fan of a one-size fits all minimum but saying this study proves staffing and quality don’t correlate is incorrect and misleading. Instead of a minimum - the amount of nursing care actually provided should help to drive reimbursement. If I’m a therapist - I bill a different code based on how long my session was with the patient. This should be the same in LTC. If we’re providing 5 HPRD - reimburse more than those only providing 2.5. It could even be built into VBP. This would work in hospitals, too! Commission for Nurse Reimbursement Incentivizing higher staffing (with a research based ceiling) will bring staffing levels up much faster than a minimum would. #LTC #longtermcare #SNF #nursing #nursestaffing #CMS #Medicare #Medicaid #StaffingMandate
