Just released, an NIH funded study, Patel et al. (2023), examining the increase in healthcare delivery from NPs and PAs between 2013-2019, among Medicare patients, indicating very interesting findings and discussion points-highlightening the value and existence of the healthcare team approach. As noted by the authors, “however, our results do not support the idea that nurse practitioners and physician assistants are simply replacing physicians in a one-to-one fashion. Rather, clinicians are complementing each other with nurse practitioners and physician assistants having a greater focus on some types of visits, possibly where their training and expertise is best suited. Across conditions, we observe substantial differences in the involvement of nurse practitioner and physician assistants with higher involvement in low acuity acute problems (eg, respiratory infections and urinary tract infections) and mental illness, and a lesser role for heart disease and eye disorders. Compared with nurse practitioners and physician assistants, primary care physicians are more likely to provide new patient visits and less likely to provide annual exams. Surprisingly, we found that in areas of the US with fewer physician visits per capita, fewer numbers of nurse practitioner and physician assistant visits were recorded. Transitional Care Management What is Transitional Care Management (TCM)? Transitional Care Management (TCM) services address the hand-off period between the inpatient and community setting. After a hospitalization or other inpatient facility stay (e.g., in a skilled nursing facility), the patient may be dealing with a medical crisis, new diagnosis, or change in medication therapy. Family physicians often manage their patients’ transitional care. TCM Coding The two CPT codes used to report TCM services are: CPT code 99495 – moderate medical complexity requiring a face-to-face visit within 14 days of discharge CPT code 99496 – high medical complexity requiring a face-to-face visit within seven days of discharge Requirements and Components for TCM -Contact the beneficiary or caregiver within two business days following a discharge. -The contact may be via telephone, email, or a face-to-face visit. Attempts to communicate should continue after the first two attempts in the required business days until successful. -Conduct a follow-up visit within 7 or 14 days of discharge, depending on the complexity of medical decision making involved. The face-to-face visit is part of the TCM service and should not be reported separately. -Medicine reconciliation and management must be furnished no later than the date of the face-to-face visit. -Obtain and review discharge information. -Review the need for diagnostic tests/treatments and/or follow up on pending diagnostic tests/treatments. -Educate the beneficiary, family member, caregiver, and/or guardian. -Establish or re-establish referrals with community providers and services, if necessary. -Assist in scheduling follow-up visits with providers and services, if necessary. She shared with us intricacies of our patient discharge process. ‘From the very moment a patient is admitted, our discharge planning kicks in. While our goal is to have patients return to their comforting home environment, their safety remains our top priority. This isn’t just a one-day effort – we reassess and plan every single day.’ Patient involvement is key to a smooth discharge. ‘We keep patients informed about their plans. If discharge is possible, we discuss their needs and organise pharmacy, follow-ups, and more. Our aim is to have patients safely on their way home before 10AM.’ Jo also explained ‘criteria lead discharge.’ ‘I This is our way of staying a step ahead. Identifying patients who, come morning, are ready to leave without further medical evaluations. With all set, some can set off as early as 7AM’ Coordinating across departments is crucial. ‘We ensure timely flow and communication, advocating quality and safety of discharge planning from day one,’ Jo added.
