Today, I had the privilege of introducing Evidence-Based Medicine to our first year medical students at Loma Linda University School of Medicine, students who are barely into their eighth day of medical school. Reiterating David Sackett’s definition of EBM as being an integration of the best research evidence with a patient’s values and a physician’s clinical experience, in this day of extreme pressures on clinicians to see patients expeditiously where the act of building a therapeutic rapport often becomes lost, the onus is on physicians to remind people who run health systems what Medicine is truly about. “Medicine” is both an art and a science and while incorporating evidence into the care of our patients is vital, having the time and taking the effort to actually listen to and converse with a patient is what makes EBM a humanistic endeavor. #evidencebasedmedicine #humanism #physicianautomomy #patientcenteredcare #shareddecisionmaking #patientsafety #qualityimprovement #healthsystemsimprovement

The Flexner report was very specific, targeted, and also devastating report. The planning and foresight was impressive, and this is how big families/family money remains in power. Planning, patience, foresight. Most people, including me, think of their own lifetime and children, but less so much about generations from now when we are long gone. The book "the Fourth Turning" goes into this, along with "Generations" and is why history repeats itself. There are some patient, powerful people who are busy orchestrating the next decades while most of us average folk go along for the ride and react to what is happening now or the near term trend rather than in the planning of those trends. Specifically the Flexner report was about better living through chemistry, getting rid of true healthcare (naturopathic, homeopathic, chiropractic alignment etc calling them "alternative" not funding their education, casting doubt upon its efficacy…) while promoting the treating symptoms with chemicals as opposed to getting to the root cause and helping people get well. We have a sick care model by design. A patient cured is a customer lost.

Grit and resilience. It seems they are NOT supposed to come easy. In fact, it seems grit and resilience are learned due to adversity. I’ve had my fair share of adversity. It was never fun, but boy do I enjoy the feeling now that no matter who or what tries to knock me down, I’m confident I can get back up again. What are your thoughts on #medicaleducation ? Phillips, Jr from The Family Institute at Northwestern University, UMass Chan Medical School, and The Center for Professionalism and Value in Healthcare discuss how the Teaching Health Center Graduate Medical Education program equips doctors to go where they are most needed; Yet, this investment in advancing medical equity is under threat. "The COVID-19 pandemic highlighted widespread health disparities and revealed the importance of primary care in improving the health of the population. A 2021 report by the National Academies of Sciences, Engineering, and Medicine reaffirmed that primary care is the only sector of the health care system with demonstrated capacity to increase life expectancy and reduce disparities. The report also identified THCGME programs as ideal training models for enhancing access to primary care in underserved areas, and the only consistently accountable model of federally funded training for physicians."
