As technology partner of the VLAIO TETRA project Happy@Home, we participated in the kick-off of this exciting project on hybrid care and smart care. We profoundly believe that the combination of intra-muros and extra-muros care is key to a successful evolution of patient care. However, the patient trajectory is not yet smooth as data is not standardized, properly shared nor integrated in the hospitals patient records. With this Happy@Home project we want to support activities towards better synergies of care by technological evolution in sensors, data platforms, dashboards and insights as earlier projects have clearly indicated the value of extra datapoints for the care providers.
