Physician Burnout, Direct Pay, and Patient Care in Dermatology 🌱🩺 Physician burnout is soaring, driving many to leave their roles, putting pressure on mid-level providers, and impacting patient care. Amid dwindling insurance reimbursements, and rising living costs, physicians grapple with heavy patient loads, complex documentation, and regulations. The result? Less quality time with patients, delayed appointments, and compromised experiences. 😓❌ Is there a unifying solution? No. We have chosen the direct pay model trying to break free from insurance constraints and regain control enhancing patient experiences. It's a shift that means more time per patient for comprehensive evaluations, meaningful interactions, and ultimately, improved outcomes. 🌟⏱️ However, embracing the direct pay model comes with its hurdles. Transitioning patients can trigger resistance, often due to cost concerns. While it might seem pricier initially, the long-term benefits are undeniable. It's a balance between patient volume and prioritizing top-notch care. ⚖️💼 In our dermatology practice, the direct pay model has been both a blessing and a battle. Two months in, we're witnessing the ease of delivering personalized care, while also facing the uphill task of educating patients about its advantages. 🌿🏥 If this resonates, know you're not alone. Day 1 ✅ After doing my IP last year I guess minor illness was a natural progression and so glad I chose One PCN Training. No matter how experienced or inexperienced you are this is tailored to everyone’s needs with discussion of real life case scenarios. A real good mix of clinicians present which allowed for some very useful conversations on best practice as knowing the guideline vs being able to put the guideline in practice are two different things. For anyone thinking about holding minor illness clinics I would recommend you making the leap and getting this under your belt. This will ensure you practice safely evidence based and contribute to ensuring patients are getting the care they deserve.

A rare case of periorbital nasal dermoid cyst 🪩 A 25-year-old man was referred to our ORL-HNS clinic due to nasal root mass with extention to right medial canthus (Fig A, red arrows). 🪩 On his HRCT scan there was a hypodense and well-defined mass at the base of right nasal bone and naso-frontal suture (Fig. B, yellow circle and arrow) with obvious smooth erosion (Fig. B, red arrows). No intracranial extension was identified. 🪩 At the theater the mass was removed in an enbloc manner with no adhesions to the adjacent tissues (Fig. C- black and white due to LinkedIn protocols). 🪩 For prevention of a hollowed out appearance due to adjacent bone erosions the tumor space was filled with crashed mineralized bone allograft (Fig. D, graft cubes showed by red arrows). 🔴 Final pathology report was a cyst lined by keratinized squamous epithelium consistent with dermoid cyst. 📚 Dermoid cysts are developmental choristoma originating from aberrant ectodermal tissue, usually develop between 3 to 5 weeks of gestation. It is an embryonic displacement of epidermis to a subcutaneous location along embryonic lines of closure. They are typically located at the points of fusion of zygomaticofrontal and frontoethmoidal sutures. Hence they are commonly seen along the lateral third of eyebrow or supraorbital ridge. 📚 Clinically, they present initially as dormant lesions, later as a slowly enlarging, unilateral, painless, firm mass with proptosis, occasionally with motility restriction, and decreased vision. Imaging shows round, well-defined lesions with an enhancing rim that may contain calcium and a lucent center. Complete surgical excision in to be the treatment of choice. The prognosis is excellent, but recurrences with infiltration may follow incomplete excision or rupture of the capsule.
