"I've got 99 problems, but time to ventilation ain't one..." ‡problem: time to setup and initiate ventilation support - speed at which cuirass application, followed by biphasic extra-thoracic #mechanicalventilation, can be instituted by physicians (wearing full anti-chemical protective gear no less!) vs. intubation ‡solution; physicians of different subspecialties performed cuirass application on an adult volunteer and instituted biphasic #BCV extra-thoracic ventilation using the RTX respirator. They compared this method with endotracheal (ET) intubation and manual ventilation on a mannequin. The performances were conducted in a prospective, crossover, randomized manner, and the times to successful applications and failure rates were recorded. ‡conclusion: The study found that cuirass application was performed more rapidly with a slightly lower failure rate compared to ET intubation. Physicians wearing full anti-chemical protective gear were able to apply the cuirass and institute biphasic extra-thoracic ventilation faster than ET intubation and manual positive pressure ventilation. take home ; The study demonstrates that ease of use, as seen with the cuirass application and biphasic extra-thoracic ventilation, results in faster implementation.
